Today we are going for MRCP planning, “how I do” it video where I’m breaking every steps into details. This video only shows how to plan the bile ducts, not pancreatic duct. In the future I will make another video for that.

Thanks for this. MRCPs is one of those procedures I struggle to get my head around. Not sure why. It just doesn't compute in my head. But your explanation and demonstration made it so much easier to follow and understand. Thank you!

Great job! I work with GE scanner and sometimes i think Siemens is better on mrcps. I guess i just don't like the free breathing technique no matter how good u position ur navigator

Hello Bac, Do u work with GE as well? If so, can you highlight more on how to use and adjust a Navigator better on GE's? What could make a navigated pre- dynamic sequence on abdomen look good, but not on the post contrast since its the same sequence ? just wondering..

Hi Tony. Unfortunately, I don’t work with Ge scanners. This sounds strange your problem mention here. Should work as the same pre post navigator though.

Hi Bac, great channel! Is there any chance you could upload a pdf with the parameters for your triggered MRCP and the bh MRCP? I saw that your triggered had a base time of around 3 min, ours has almost 6min, would be great to try out yours.

Can you please tell me the software version you are using here? I'm in clinicals and the hospital I'm at uses this with their 1.5T Aera and I'm not familiar with the icons and their meaning. The tech there doesn't know what version of Seimens software it is. I was hoping to find info online with no luck. Thank you for doing these awesome videos! It helps me a lot!

Resp trig takes everything from 2-5 min. Depends on patients breathing pattern. Bh is used with caipirinha.

Can You please tell me for BH space Turbo Factor, Base, Phase and Slice resolution. Caipirinha Acc factor 3D is 3 with 24 ref lines? Amazing images 👍

Etl 240, base 256, phase 80%, slice 50%, caipi 3 24 lines. My goal with this Bh is not having it isotropic. Just sharp in one plane

Hi Bac. Thank you for share your knowladge to us . I have a question. I work on siemense 3 T in liver transpalate center . I have problem in 3D space . Becuase my patients have asitise . If i change TE to 400 in 3tesla is it work? My protocol take 4.30 s in 3 Tesla i dont have a short TR

There are many key factors for having a good mrcp. Firstly is the patient patient. Secondly is the technical part along with coils and patient positioning. I use te around 400ms.

Thanks. This approach I used was liver dome. Most standard one been for many years and on older software. Then you have what is call phase scout which is the more newer version of navigator.

Anh ơi, sao có những lúc em chụp MRCP lại chỉ tăng tín hiệu của đường mật, còn túi mật lại không có tín hiệu ạ. Trong khi ở xung T2 tra lại thấy rõ túi mật căng. Em cảm ơn ạ

Good question. In general the physics behind 1.5T and 3T are different. So yes I do that.

Bac . I have been trying to set a protocol for a breath hold MRCP. I tried to copy the protocol you had but I always end up with a 1 min and 32 sec. time breath hold. I adjusted all the parameters but I can't make it to a 15 sec. BH. Please help. Siemens Espree.

Bac, I notice on your t2haste and 3dspace you drop you TR, I believe the original TR on siemens default is 1400 on haste and 2600 on 3Dspace. When I scan I don't bother adjusting those TR, will there be a significant changes if i drop those TR beside Time acquisition?

Good notice. 1. Haste is a “hard sar” sequence. So having a high tr will cope with this. However I don’t want to have two breath hold in one sequence. I drop the tr and drop the flip angle so I’m able to do this a one breath hold without any jumping in between those two breath hold. Not a single person will be able to do the exact breath hold position of the anatomy if you ask them to do it twice. That’s why images often jumps on haste because you do it twice in one sequence. 2. For mrcp 3D space. This sequence is very complex whenever optimising. By that I mean you can not only change tr alone or te or etc etl.Many parameters must fit with each other for being able to highly optimise this sequence. There are many holes of pitfalls you can end up with if optimising this wrongly. I drop tr to save time but other parameters are adjusted accordingly to that.

1. From 1400 TR to 500, will there be huge impact on my T2 contrast? I always wanted to do single breathold on my haste, When my patient barely follow breath hold instruction I end up resp trigger haste. 2. good thing I know I can drop my TR on these, trial and error I guess + review on the basics. Even with a good respiratory gatting, I have mediocre quality of 3Dspace full of breathing artifacts, probably because Scan time takes 5min+. Thanks a lot, will try to improve my mrcp protocol.

Regarding haste. No. It’s a single shot type of sequence. All good by doing that. Nevertheless, haste is also a good sequence for practising the breath hold.

Bac, follow-up questions 😅 Did you raise your turbofactor on Breathold MRCP? Can't barely achieve 15sec scantime without touching turbofactor.

@Bac Nguyen ohhh, ok. But you read these clearly right. How accurate are mrcp images ? Like I said, I had one done, with iv contrast, and without. They found nothing. Just looking for second opinion. I had a CT scan with contrast back in the summer and a full mri body scan in November. All negative.

@@antoniocognato5023 I don’t read these. But I can see if the quality meet its need. Different modality such as mr, X-ray and Ct have its pro and cons. For bile/ pancreatic duct mr is good. Highlight the biliary pancreatic duct nicely.

I think its way over complex we have much easier breath hold radial that covers all ducts biliary and pancreatic
