I just triggered this. This is my 15th run or so, and I have never seen this before. The only thing that changes is that my last playthrough was as Karlach Origin, and the end wasn't good.

@@Asteroidhawk594 I don't know for sure. My friendship with her was Neutral. I had just finished Shadowheart's quest. The only thing I did that was different, besides being mean, was talking to each companion after every mission.

@@spamaccount8979 I Play on PC, I have a lot of mods, but, I don't have a specific mod that enables this, most of my mods are for making the game harder and for cosmetic aspects.
