Sometimes the meat falls off the bone because the ribs had been cooked under a low and slow oven for at least 2 hours to tenderize the meat first then transfer to a charcoal grill for extra flavor and color. I understand that due to cost efficiency, some restaurants would skip some steps and the result is obviously noticeable.

I agreed with you. Personally speaking I prefer meat falling off the bones much more.
