The most poignant part of the song (all of 'em, actually) is the self-awareness. It becomes raw because we can all feel it. It's not like someone going crazy in the abstract, that they don't even realize they're out of control and don't care, there's a degree of separation that makes it easy to write someone like this off. The fact that moments of clarity allow for this recognition closes the gap and makes sympathy easy. If you see someone masking their hurt with anger and hurting others, it's easy to go into self-defense mode and just stay out of the blast area, come what may. But if you see someone hurting, crying out for help and wanting it to stop, it's natural to want to jump in and help if you can. The lyrics hit home every time.
