Excellent lecture. Can somebody please explain why the RA is visualized at the bottom left of the picture when looking for the IVC? I can't seem to wrap my head around the connection between the ultrasound probe and then image that is generated in that particular view. Thanks!

Two simple points/rules to help you orient yourself for any image: 1 - The top of the screen is where you probe is touching the patient. The bottom of the screen is where you are aiming/directing the probe. 2- The probe marker points the same way as the probe indicator on the screen (dot, company logo, etc). So in an IVC view we have the probe on the subxiphoid area with the probe marker directed towards the patients feet. Because we are in a cardiac setting our probe indicator is on the right side of the screen. So using the above rules we can orient ourselves on the screen. The top of the screen is the patients anterior abdomen. The bottom of the screen is the patients posterior or back. So the ivc is at the bottom of the screen. 2nd - our probe marker is towards the patients feet in this exam and we are in a cardiac setting. So the right side of the screen represents the caudal portion and the left side the cephalic portion of the image -- This then puts the heart towards the left side of the screen (cephalic) and shows the IVC dropping in. I hope that helps. Here is a video I did a while back on b-mode ultrasound that goes through those rules. Keep in mind that this is in an abdominal setting and the probe indicator on the screen is moved. The same rules apply though. kzbin.info/www/bejne/qHqcpGppqtSpiKc

This was great. Can you clarify something please? Can this method for estimating the PVC be used in a spontaneous breathing patient or must the patient be intubated?

Did you mean CPV (central venous pressure)? The technique taught in this video relates to the spontaneously breathing patient. Often if they have normal healthy lungs you won't see a lot of variation likely because they don't have to create a significant negative pressure to breathe in during inspiration especially in a supine position. I like to tell patients to to sniff in quickly like they are smelling a flower. This will show respiratory variation quite well.

Just was in the icu and when I walked in the first tine, my gram was knocked out and the nurse was just beginning to do this. It was amazing to watch a heart do it’s thing, and it was my gramma’s at that! so I was in the back corner in my chair peering over the nurse’s should to watch the screen and try to get a grasp on what I was looking at. Btw I was gonna be there from 8am to 8pm most likely so I dosed with my med cannabis. Hey, docs orders! So I’m in a zone watching the screens and she was now on her 3rd or 4th screen when I saw the hearts valve open and shut. I contained my voice level and asked the nurse if that was her heart valve? The nurse said yes and seemed happy to have a diff type of convo from her other patients and co workers. My gram is overweight with diabetes and had a stroke about 8hrs earlier. Smoke her whole adult life but never had a serious heart problem but just comparing her valve strength in movement compared to this videos example of a healthy one was night and day. The whole heart ‘show” last bout 20 mins and was very interesting. I’m her grandson and was her main support person and my one aunt was the other. Because of covid, only 2 were allowed to come and same two. Must be one at a time too. Gram never woke up and knew she wouldn’t want to try to rehab in a nursing home and stuck in bed. That was her nightmare and I promised i wouldn’t let it happen. It was though, but in my mind, thankfully my gram showed no signs of improvement and I had no qualms to start getting my 4 aunts and my father to prepare for hospice. Next day we took her off her ventilator and she passed peacefully 20 mins later. Everyone was crying. I was relieved. She had horrible back pain for the last 20 yrs and for the last yr, we had to finally jump into the opiate world and simply make her life more comfortable. For the 1st time in 20 years of living with my gramma, I finally saw her without pain. I didn’t cry that day and haven’t cried since. Of course I miss her but I would feel selfish to want her back here and be hurting all over.
