Distal bicep tear (complete) left arm back in Mar (2024); repair surgery a little over two weeks later. Cast, then brace (each for a few weeks). I have a few more months until near complete recovery; however, I have full range of motion now and some strength back. What help significantly, was working on my range of motion soon after the surgery. I did not take any pain medicine (post-surgery)…it subsided after about a week. I participate in physical therapy once a week. Both the [excellent] orthopedic surgeon and [excellent] physical therapist are kind of amazed at my progress. To everyone experiencing this, please keep a positive mental attitude…you can overcome it and get back to your normal life. The same mental strength helped me back in 1986 when I almost lost my leg…I was able to complete the remaining 12 years of a 20 year career in the U.S. Army. I am 3 weeks post surgery, mine was half-torn. They did the cut and repair, with the button drilled into the bone. The plan my Surgeon gave me is as follows -> 2-weeks sling, 2-weeks in brace at 50Degrees range of motion, then 2-weeks in brace at 100% range of motion, then remove brace and start light use...

Just recently flared up from all the labor I did on my home. Haven't slept well for months. Finally went in and had it looked at. Nice to hear from someone who lifts that's had it done. Thanks.

Don’t be depressed, Andy. Your bicep might break but your mind and your will, cannot. You’re cut from a different fabric brother, you just have to remind yourself that. When a negative thought enters that mind of yours I want you to turn around and look around your gym. Look around your home. At your kids. The accomplishments you’ve made. Life happens dude, is this the first time you fell off the horse? Certainly won’t be the last. But you already know you’re gonna get right back on that saddle no matter how big the horse. You don’t need “get wells”. You’re already well. See you on the saddle.

Mate-I've just seen your other video but this one is more apt for me. Exactly the same here! My shoulder looks like when I was 13! Complete muscle wastage. I'm 6 weeks out. I walked 32 miles the other day. Just gonna get legs like tree trunks and give it 6-9 months before I fully start with the arm again. Thanks for the video

I stopped doing deadlifts months ago due to a case of golfer's elbow so bad there was no way I could deadlift with it. It hurt so bad in the beginning I couldn't hang a 5 lb bag of frozen chicken on that arm to carry groceries in. I'll never forget that. Then I reached the end of my LP and went on to the HLM phase for cutting like you recommended and began to use this as an opportunity to cut some fat off while the elbow healed. Well only doing pushing work and NO pulling work brought me out of balance and really exaggerated my TOS (Thoracic Outlet Syndrome) and I've had to just stop everything for a while. I'm using this still as an opportunity to cut as much fat as possible and put my energy into other extra stuff that needs to be done anyway. Your Barbell Prescription book really helped me as a master lifter because it stressed the fact life would get in the way for me and I would be going back and forth between NLP and intermediate. That's exactly what's happening. I'm thankful for the warning! Sometimes life just throws us a curve ball whether we like it or not and we have to make lemonade out of lemons. Your videos and written knowledge have helped me a great deal. Thanks for sharing them and get well soon!

@@Cheers7807 doc doesn’t want me to extend all the way yet. Currently at 30 degrees before it’s all the way, I feel like I prob could too. Mobility is great, I find myself taking off the brace and doing normals tasks w/o picking heavy stuff of course. A bit tight on pronation but other than that getting better everyday. Just jogged 2 miles to stay upbeat

Just had this surgery done August 25th 2023. Endobutton was still used but they encouraged me to move my arm. Within reason, no weight but whatever range of motion was possible. They actually discouraged me from using a sling unless I really needed it. I'm 3 weeks post op now and starting physiotherapy tomorrow

I stopped wearing the mechanical brace 2 days after the Dr unwrapped my arm which was 2 weeks after surgery. 1 month post surgery today. Fortunately for me the surgery was on my non dominant left arm so it has been easy to turn it off mentally. Strained it once lifting up a baby way too soon. Feels great with a full range of motion. I still have numbness around my thumb. I will wait another 2 weeks before doing any weight training as in even a 1 pound weight. Arm works and I want to keep it that way. I am retired and don't have a need to use my left arm for anything strenuous. My greatest fear is to damage the tendon at the repair site.

It's funny, I've had the surgeon and the hospital physio tell me to get full rom asap. I'm on day 9 since surgery and can move it all the way up and down

rumplespliffskin same here. Had my surgery a week and a half ago. Tore my tendon off the bone at the elbow right before my beach vacation. Luckily my surgeon works in sports medicine. Only had to wear sling for 2 days. Went to the beach 3 days after surgery with no sling or anything. Just bandaged up. Surgeon told my to lightly move it up and down. I can almost totally straighten my arm now. Still sore but I can’t imagine my arm being in a sling for over a month. He said he don’t believe in being in a sling for 4-6 weeks.

@@Smokeshow767 I was told by the doctor who referred me and a surgery nurse that I'd be in some sort of a cast for a month. I thought I was going to spend 6 months trying to straighten it again. Right as I was being knocked out, the surgeon (specialising in sports injuries) said he wants me gently working at it as soon as I wake up. The physio said the same thing the next morning. I'm 5 weeks post op with an 8mm hole drilled in my forearm and in my experience, walking around before you can straighten and let it hang it is the only reason for a sling after a day or two. Just don't pick anything up or do anything stupid like curl with something in your hand.

I had my surgery about 3 weeks ago using the same procedure. I was out of my cast in 4 days and started occupational therapy on day 4 as well. I’m still in a sling when I sleep and part of the day. My surgeon told me to wear it to keep me honest so I won’t use my arm accidentally. My OT stated that the endo button is extremely secure and that there is no longer the need to keep your arm immobilized for a long period of time like with the previous bicep repair procedure.

@@edwardsg1 I forgot about that. I'm nearly 16 weeks now and it's amazing how much of it is a fading memory already. But yeah, I actually kept it in a sling when sleeping for a week or more. It took ages to find a comfortable sleeping position as while the endone would knock me out, I'd wake up a few hours later with a really sore neck or shoulder from sleeping on my back with my arm raised on a pillow. I only used the sling to walk for the first few days although I used it for the park run 10 days post surgery. Once it would hang limp while walking, I just walked carefully. It had a massive bandage so everyone gave me a wide berth.

Hopefully this will ease everybody's mind as you can see from my picture I'm in a wheelchair I missing both my legs I tore my rotator cuff a SLAP tear on the labrum and I tore my bicep anteriorly off the bone. I want the Mayo Clinic have the best guy in the country do the surgery depend on my arms for everything not having legs. One thing I'll be honest about my rotator cuff didn't really hurt after surgery my bicep tendon had me damn near in tears after surgery. It it's a pretty tender surgery. The good thing is after three or four days I was feeling okay pretty much back to pre-surgery tight pain right now I'm on day number 6 period if a guy with no legs wheelchair-bound can you get through the surgery you can also no problem. One thing I would recommend as if the doctor says a lettuce for the pain catheter or let us dead in your shoulder before you go home jumper pain. Let them. It is a huge help so is a ice machine

Just yesterday I was looking to see if there were any updates on the surgery. Glad to here that the repair went well! A few years ago I dislocated my shoulder and had the same feelings about not being able to train. When I was able to start back it was with just the bar and even that was painful. It took about 8 months to recover and get back to where I was. The doctors will be overly cautious and want to wait way to long. It will take some common sense and listening to your body during recovery, if you do you will know how hard to push. Make sure you get full range of motion back before using heavy weights that will want to stretch it out.

Almost 1 month post op, but 3 months post injury (210 kg rack pull...would have been PB). Had same type of surgery, but got a haematoma and infection once the staples came out. About 30 degrees shy of fully extending my arm right now. It's very tight. My left arm has also atrophied considerably. Really missing the gym. Considering doing some light resistance band work - working around the bicep. Maybe some flys and bent over rows, if I get the go ahead. You're still a beast Andy - thanks for vid.

@@brantcollins4115 I went to a training hospital - not sure they did a great job tbh. My bicep is noticeably bunched. Strength wise, maybe a very slight decrease. I've heard others say their arm returned to normal, so maybe worth asking around some more first. I also occasionally strain my forearm muscle (brachi) and need to do physio to reactivate the bicep. Not a huge deal, but something I will have to deal with. Again, this might not be an issue for you, should you decide to go ahead with op. Maybe consult with a physio first, if you haven't already. Sorry - not much help. Best of luck with it!

You should be in splint 2 weeks and then adjustable brace for 4 weeks according to my surgeon. I just had surgery yesterday. I thought I could switch to Tylenol this morning. Had only had Tylenol today. Now it’s 6pm and my arm is hurting more than it has since I’ve been home so I just took another Percocet.

@@stephenstrutt6615 at week 6 I started 1-3 lbs. Yesterday was my week 8 mark and I moved up to 5 lbs. Starting at week six I probably started to overdo it with just general use stuff and it would always tell me. I’d say the camera is definitely going to be too much for at minimum the first six weeks. At week eight I feel much better than I expected. Just be patient if you can.

Watching all your vids Andy. Coming up on 2wks post op. Training legs - no safety bar squats yet but vertical leg press in my garage too. Left side incline chest db very light, static holds, contracting and squeezing both pecs - left side curls pressdown cable rows Scapular stuff - no weight on right side -- Brace 90 - 60 degrees still Doing slight resistance against other arm neutral grip bicep flex (very light pressure keep contractions short and right arm at 90 degrees but harder press at 90d for triceps Ugh-- it’s tough but I want this thing to heal - arm looks like a 15y old girl already smdh.

I am appreciating all the extra content you've been putting out. The past few blog posts have been great, particularly the last three. Are you planning on writing any more programs? I realize you just released a classic HLM I haven't picked up yet, but your past few blog posts have got me hoping for a 4 day powerbuilding program using %'s. Basically a hybrid of your KSC Method for powerbuilding/powerlifting. I love those two and have been rotating between them since January and despite dropping 20 lbs I have been setting PR's about every 5 or 6 weeks. @@brantcollins4115 hi man. God be with you. 3 months no training. From 3rd month to 9th month you gonna have to take it very slowly. Gradually raise the tension of training. During 12th month almost all will be like before. After month 12 you will even start develop. First 14 days will be hell. After 14th day will start physio therapy until 3rd month. First 24 hours will be easiest due to pain killers. Stay in the hospital for 2 3 days. Good luck.

Tore mine Memorial Day weekend, >50% partial tear. So my bicep did not retract. Doc originally wanted me to wait 6 weeks to see if it healed, after about 2-3 weeks I pushed for surgery (endobutton), had it in less than 4 weeks total even during all this Covid. My surgeon had already told me all he did during the non elective surgeries were distal bicep repairs, because those are essential. Did 1 week in the weird brace/cast they made me, and I begged out of it at 1 week post op and got the hinged brace. They set it to 35-125degrees motion when I got it, I decreased it cause that didn't seem right and I wasn't ready for that yet. Just had my 2 week post op follow up, doc basically told me motion is fine, just don't pick up anything, and wear the brace when I need to be reminded not to do anything dumb. Follow up 4 more weeks away, and he doesn't foresee the need for me to have physical Therapy. Said I can just do it on my own at home. I have almost full extension already, but flexion is an issue, and supination is a big fat no with some pain. Still a giant knot in my arm that I am to massage out. One incision, a big one, right below my elbow on my forearm. They used surgical glue, so no staples or stitches for me. Hope it works out.

@@TheSantos173 didn't really need any therapy for it. Once it was repaired and I hit the 3 month mark, doc cleared me for anything that I could physically do. Feels fine, as opposed to my ACL repair that happened a few months after this surgery (that one you really need to do the therapy, and then some, I am still not 100% after almost 2 years on). I can lift and do everything with my bicep, although I feel a bit of a twinge at surgery site when throwing a baseball hard, but maybe that's just old age, and lack of a lot of throwing until recently.

Similar issue. Recovering well. There is some deformity in the lower bicep. Any exercises I can do to strengthen that portion of the bicep and eliminate the deformity.

@@taskmaxter I went in for the op on 2 May 2023. 3 weeks after the injury. Probably waited too long. Also trained, including curls with the severed distal tendon. I was trying to convince myself that it would be fine. Soon realised that it wasn't going to be fine. Started to feel pain in the tendons on the other end.

@@taskmaxter the arm before the op started to look worse and worse. The bicep bunched up towards the shoulder. The surgeon reattached it using the endo button. No cast, just a sling for 3 weeks. Didn't lift anything heavier than a coffee cup for 3 months. Whole arm atrophied. Only started rehab with weights of 5 kg on 1 September. Added 2 kg of weight per month to bicep curls. Jumped to 16 kg this month. Arm looks and functions WAY BETTER than before the op. It really was deformed in comparison to the other. There is still still a bigger gap between the bicep and elbow and the bicep is still smaller and weaker but the difference decreased every month. I don't think that it'll ever be the same as before, but it has a great peak now due to the slight retraction 🤣 gotta try look at the bright side. If you can, DEFINITELY go for the op. Sooner the better.

Do what your surgeon says, man. I'm not a doctor....I'm sharing my own experience here....not to be confused with actual medical advice. There wasn't a lot of info about this online when I tore mine nearly two years ago so that's why I chose to make the videos.

@@andybaker2413 thanks for the answer! appreciate it. heres a stupid question. if i put my arm in a tight sling, do you think its dangerous to ice skate. if i fall there will be no danger of me extending my arm, so i dont see the danger.. what are your thoughts

@@joelreader6151 I wouldn't. But that's just me. Falling to that side and landing elbow first on the ice seems like an unnecessary risk. But I live in Texas so we don't skate much here. :-)

thank you for your time, your video is really helpful. i m 48yrs and i had this surgery 1 week ago. my questions for you are: when was the first time you got your elbow straight (180 degrees)after the surgery ? how are your supination and pronation movements after the surgery? one day after the surgery i had a meeting with one of the hospital Physiotherapists and he ask me to start practice some specific forearm movements right away when i get home and some weekly goals, ex: after the first month i need to be able to extend all my forearm/elbow.180 degrees... but in your case , the advice was 6 weeks without any movement? thank you again 👍

My surgeon had me in a cast for 2 weeks. Then a mechanical brace. Every week I would increase ROM by 10 degrees. I'm 5 weeks out and at 30 degrees extension. Should be fully extended in the next 3 weeks. I'm doing my own PT with isometric, then zero weight flexion, then 2.5# increments until February 1st. I should be curling 20# by 2/1/25. I'm not sure if I'm going back to full strength training yet. This injury sucks. Might focus on body weight exercises and ROM exercises. I'm getting to old to be lifting heavy and sustaining injuries like this.

It’s concerning to see the widespread spectrum of methodologies for post surgery recovery. My doc had me on splint for only one week and then a brace for 5 more weeks, increasing the angle every week. However, I was on PT start of week 2, with isometric strength exercises as well. I’m at week 4 and feel great. I can supinate all the way and stretch out my arm all the way as well. Doing a lot of grip exercises and started isometric bicep exercise on week 3. I’m anticipating cutting that 4/6 month recovery in half based on my progression.

I feel you. I think not all doctors for this procedure are the same. My doc is great. Started pt 6 days post surgery and range of motion. No more than 30 degrees

I was un-lucky enough to blow both arms at once! Problem is that I waited for over a month before going to the doctor and now have to wait for a surgeon to repair both! Thanks for the video and information!

I live on the East coast of Canada, and during one of our last snowstorms our car was stuck in a small rut on some ice. While my wife was giving the car gas, I was in behind try to push it past the rut as the car was spinning. The car is low to the ground and as I was pushing but at the same time lifting, hoping to get it over the hump. Suddendly I felt pops in both arms! I have been lifting heavy weight most of my life and the doctor thinks it may be as much the accumulation over the years as the final act when they snapped? The strange thing was that I really was not exerting an extreme amount of force at the time! However I am 49 now..lol

Hi Andy, why can you only do "light movements" on the leg press and curl? Does training heavy interfere with recovery of the tendon? I followed Brian Alsruhe's recovery on his rotator-cuff tear. He trained with his sling on, doing heavy neck strapped deadlifts and SSB squats. The day he was cleared to lift he deadlifted 700-something lbs off the floor. The doctor argued he could not lift more than 2lbs until he was cleared, after that he could do whatever which just sounds silly. I know it's a different injury but I think it's similar in recovery. Best of luck, looking forward to the rehab series.

This video is odd. My doc is pretty big into this technique used for this injury and I was told to start PT 6 days after surgery. What kind of doctor says pt isn’t necessary? More so why was this guy told no movement at all for 5 weeks. You need to slowly get that tendon stretching along with pt for scarring. I’m so fortunate to have a good doctor and not what this guy had

It's a permanent fix, to the degree that anything is actually "permanent" But it'l last a lifetime and allow you to resume all activities.

Hi andy I popped my left elbow tendon two days ago I’m also 36 years old I am exactly like what you were saying in the video watching your story two years later the problem is that I live in Shanghai and I have yet been able to find a good surgeon in the area contemplating going to the US to get it fixed but the ongoing pandemic is not making it easier could you please recommend me to your physical therapist New York I will try to get it fixed in China over the next couple of weeks hopefully I’ve been searching all over China for a good orthopedic surgeon with experience in this operation

Well I had the surgery yesterday. Had the double endo button, on the top and bottom. My question is did you have wrist soreness? I have the soreness at the bicep and the repair spot,but it feels like my wrist was spun around. Mr doctor wants me to move my fingers and waive the hand ,but stay in the sling. I just curious.thanks for the videos on this subject.

Yeah man. After the surgery, everything is sore and hurts! You've had a lot of trauma and shit is inflamed. It gets better with time. Follow the Docs advice for first 6 weeks. Move the wrist and shoulder around as much as possible, but just be careful with the bicep/elbow itself.

I’m currently one week out of surgery postop and I’m wondering if anyone has had trouble opening their hand, my fingers are very stiff and I cannot move them an example would be as if I was counting using my fingers 12345 and I cannot open them without using my other hand. Has anyone else experienced this? I had a complete rupture of my distal bicep tendon they used a button technique to repair

I had a lot of issues with grip strength after surgery and general stiffness but I'm pretty sure I was moving my hand and fingers pretty well 1 week afterwards. This might be a good time to make sure you get in and see a physical therapist. In general I'm not a huge fan of PT, but in the immediate post op window it's probably a good idea, espeically with an issue like this.

@@beast8995 hey I’m sorry to hear about that. I am not currently having these problems anymore it took 4 months of physical therapy to regain 100% movement back in my hand. I thought it would never come back and it was scariest thing ever but have patience, go to therapy and do what they say and it will come back to you. The nerves need to grow back and repair themselves. Takes time but it will work. I wish you best of luck anymore questions don’t hesitate to ask, I was once in your situation it gets better.

I just had distal bicep surgery....what's interesting is my doctor told me not to use the sling after two weeks but I was out of the sling for one day and my arm was hurting so bad I just said screw it I'm keeping it in there for another four weeks but after hearing what you had to say I'm wondering why he told me to go ahead and not use the sling.... So I'm going to keep it in a sling for 4 more weeks but thanks for your video man at least I know that other guys have went through this I am a former bodybuilder when I was younger but I ended up getting this injury just a lifetime of heavy lifting and pushing and pulling and my most recent job is brutal and being at 51 it eventually caught up with me so thanks

100% fine. The only last consequence was that I can't really do supinated grip chin ups anymore. I CAN, but supinating that far over is just extremely uncomfortable. So I just use neutral grip or pronated grip - no big deal. Everything else fine.

For the most part I like Joe D and I've stolen many ideas from him over the years. I don't agree with his approach to training young novice athletes. Novices have a window of time where strength can be increased very quickly and I think it is a mistake to not take advantage of that. Taking a young kids squat from 135 to 315 will get him faster than speed work, plyos, etc so I say drop all that shit and focus on strength. As the athlete progresses in strength you might scale back frequency a bit and then i think a lot of his ideas make more sense. Working with athletes is challenging because you have to navigate your workouts around their practice and competition schedule and you may only get 1x/week under the bar with them. If you aren't a competitive athlete doing sprints and all that stuff on other days of the week, then I'd have most people doing lower body 2x/week

It wquld depend on your level of strength. You don't need to squat 500 to play basketball, but I'd want to get to say 315 or so just to adequately protect myself from injury. Getting even higher than that can maximize whatever potential you have for speed and explosiveness. If you are already there, then fine, 1x/week is plenty for maintenance. If you are squatting 185, then yeah, you got some work to do and I'd suggest 2-3x/week lower body training

@@brantcollins4115 I did it and im back at kickboxing since last month and I do small weights to get it back in shape since day 100 post op. So far so good but I was literally lifting 40lbs whit my fucked up arm around 20 day after I tear the tendon. I did the surgery cuz im Canadian otherwise fuck paying 10k for that imo. Im 25 and lowkey pissed af that I missed 3 month of training and my arm is still weak but its just getting started on the strength rehab. It should be stronger im focused on the long term cuz im young.

There's no way that the bicep will stay the same after the total rupture. I've seen some content on KZbin that it can. Maybe those guys are genetic freaks. I'm so scared. Even with this tendon dangling around in my arm and a bit of bruising, I'm able to do 100 kg bench and 18kg curls. It's freaking me out not being able to train for 3 months because it's the only way I stay de-stress from work and life. Hope the arm doesn't wither to an overcooked noodle too quickly 🙏 how did your arm hold out during the inactivity? Did it atrophy quickly?

The thing with Brain Shaw is A) he's got a team of pros in the field around him, B) Brain Shaw after surgery was what a normal bodybuilders would be and his muscle memory would help him jump back to enter the world's strongest etc.
