I'm glad you're properly playing the henry stickman collection blitz by finding all the fails and endings but you can use the map at the bottom left to go through all the scenarios and make at easier to find all the fails plus doing nothing when there's a timer counts as a fail plus there's a lot of tv shows and video game references in this game so try to catch them all *wink wink*

The other fail is when your running from the two cops, the first time you got all of them but the second one you didn’t run out of time on it, running out of time counts as a fail.

I mean 100% opacity means "not see-through at all", and "opaque" color means you can't see which color is beneath. it is misleading, but technically not wrong. being as opaque as you want could mean not at all (0% opacity). but yeah, i think maybe they did get mixed up, otherwise they would've said it better.
