good video. in our setting we use one sterile wipe for each culture bottle and clean from the centre of the wipe then the four corners of the wipe.

Several large hospital chains in India have already junked the plain syringe-with-needle approach.. Also, certain laboratory chains... However, the piecing is directly with a needle attached to the adapter... And of course, the vacutainer bottle is clicked into the adapter

Unable to see in your demo but I assume your blood system components were sterile before use? I.e. the connection item between butterfly and vacutainer bottles ? Here we use very strict method to ensure aseptic technique, and collect samples from two different sites (so, 4 bottles in total). Guess this will vary according to institutions’ protocol

Just want to ask if it's necessary to wipe the top of culture tubes upon uncapping, when it is already sterile upon manufacture?

That is a very good question. At work we are told to do the same by disinfecting the tops.

The reason is because just in case something went wrong at the factory and it's not actually sterile, this serves as a sort of final insurance.

Us phlebotomy students are told to go ahead and sterilize the tops of the bottles. I guess even in some hidden places, germs are lurking!

He’s working toward being an attending in emergency medicine in the USA, so I can confirm he has passed finals many years ago. We’ve stuck to saying final year medical student to keep things consistent.

Geeky, should the torniquet be removed after a flashback or only after the collection is complete? I was taught to remove it right after flashback.

This is for IV placement, for blood draws you leave the tourniquet on so pressure is applied naturally pulling the blood into the vials/culture bottles. If you take longer than a minute, then you’re supposed to remove the tourniquet but it may take longer filling the vials using only the vacuum seal.

Can you collect a blood culture sample from an existing line that was used to give drips?

Depends on your hospital’s SOP, but generally no. Lines used to administer medications/drips (i.e. IVs) are considered contaminated and will therefore produce a false positive result. However, sometimes BCXs are ordered to check for infection in more permanent lines such as PICCs, Midlines, ports, etc. and they will be drawn from. Ultimately it depends on what infection you’re checking for and where you think it may be.
