My statistics: over 9 years boarding on average more than 1 vessel per week in small bulk terminal experienced 2 pilot ladder breaks.

I watched my division's LTJG get crouched and dunked when he tried to board off a Coast Guard small craft in 10/15 foot swells. He was alright just a bit embarrassed. The Cost Guard coxswain was brilliant. He held his boat solid against the ship with no slamming. He probably save the officer from serious injury. He was way better than our ship's small boat coxswains. Some people (including me) call them jacobs ladders but the proper name is pilot ladders.

This pilot did not appear to be medically fit enough to climb that ladder.

He is not looking like a pilot, probably he's a crew member joining and it is normal not to have the same experience and tranquillity in climbing such a ladder...!

Yep as others said he does not oook like a pilot, more like a crew member or technician joining. Didn’t look like a pilot boat either! If climbing pilot ladders regular then you learn to do it fast!

Once the pilot is on the ladder the pilot boat always immediately moves away a little distance. Because if the pilot falls from the ladder: 1. He will fall into the water and not onto the hard deck and structure of the pilot boat. 2. If he didn't fall onto the pilot boat, but fell into the water right in between the pilot boat and the ship, there's a risk of him getting crushed in the water between the 2 vessels. 3. The pilot boat will be able to see more clearly where he is in the water to rescue him.

A personnel basket could be a safer way for the Pilot to transfer from Pilot vessel to ship and visa versa. A crane on the ship could hoist and lower Personnel basket, with Pilot safely.

Only in good sea conditions. In rough sea conditions, especially at night, they'd be useless and more dangerous. It also takes all the control and decision away from the pilot and instead puts it all on the ship and its crew. The pilots have to board random ships with random crews from all over the world. Including some parts of the world with very poor standards in ship machinery, maintenance and safety and in crew competency and safety. No pilot will be willing to trust the control and safety of their life to a strange ship with a crew of strangers the pilot doesn't know. The pilots much prefer to have entire control over their own safety regards boarding and disembarking.
They Could design s safty harness to attach an freely slide up along side ladder-

No, they can't risk being attached to and thus impeded and/or trapped on the ladder in any way. There are potential scenarios, especially in very rough sea conditions, where they may need to immediately split second re-position on the ladder or jump off the ladder.

Non-compliant combination arrangement. An officer with radio must be present at the combination ladder. (Not watching from the upper deck meters away.!!)

I think the pilot would be better off without the "assistant". Assistant looks like he is going to push the pilot. No way can the assistant hang onto the pilot with one hand so what is the purpose of him being there.

@@alexfx7500 Alex - just think about what you said. The assistant must be a mega body building with a right arm like an ape to be able to hold a man who was falling. If the pilot were in the water, the assistant could not reach him unless he had arms that were 6 feet long. Pilots board ships all the time from pilot boats without an "assistant" who really does not do anything. The assistant is somewhat like a conductor on a freight train. Union rules or similar. Case closed.

@@alexfx7500 I grew up on ships. My father was a licensed captain and qualified as a pilot. Most pilot boats do not have an "assistant".

@@dishenharilal4437 He gets to the top by transferring off the ladder onto the walkable gangway that's positioned right next to the ladder at its midpoint.
