The twist is that the yandere Micchan isn't the alter ego, Amamiya is. Micchan and Teru were falsely accused and ostracized in kindergarten. They became close and Micchan gave him lifetime promise wish coupons for his birthday. Instead of using them for their intended lewd purposes, he used one to wish that he didn't have to go back to the kindergarten anymore, so she burned the kindergarten down with everyone in it. Terrified by what she had done, Teru used another coupon to wish that Micchan would disappear. The heartbroken Micchan tried to keep her promise first by stabbing herself in the chest with a pair of scissors, but the chest wound didn't kill her. Instead, she became Amamiya who hated Teru, was out to get Teru, and wasn't bound by the promise, thus Micchan made Micchan disappear to keep her promise, but Micchan would still emerge whenever Teru is in danger in order to keep another wish promise. In the end, when Amamiya was about to kill Teru and he called her Micchan, she told him that she wasn't Micchan and wasn't bound by promises. She said that Micchan didn't exist anymore and that he made a wish to make it so. Before Amamiya could kill Teru, though, Micchan emerged and shot herself in the chest to stop Amamiya. Teru called for help to try to save Micchan, but she told him it wasn't necessary as she had to finish off Amamiya to keep him safe and she herself had to disappear to keep her promise. Teru told her no-can't-do because he still had a lifetime wish from her left and wished for Micchan to stay herself.

dawg the story ends on such a fucking cliffhanger, for those who want to know basically, micchan was real and amamiya was the real villain, micchan(or maybe amamiya, its confusing) also burnt down their elementary school killing his father and another girls parents, but it was actually because he asked her to do it, it ends with amamiya trying to kill him only for micchan to turn out to be real(the last couple of chapters it revealed that everything "micchan" did in present day was really amamiya, so it seemed that micchan wasnt real) and shoot herself in the chest, then he walks off carrying her and thats how it ends
