I really like seeing doctors, but once I fainted when I heard "bad news" from cardiologist. Clinical breast examinations can detect a lump in your breast and other changes that might require more testing. Breast exams are one of the most important early breast cancer screenings.

It’s because of the lymph nodes that are located there, if you have breast cancer I know that that’s the first place they check to see if it’s spread further than the breast tissue (not a medical professional but my mum had breast cancer) I guess they’re checking to see if the lymph nodes are swollen?

So James you mention at the end no obvious lump was found, in this situation what would you do? Would it be appropriate to request the patient attempts to locate the lump themselves and demonstrates such to you? Or if the patient is still unsure despite no lump being found, a possible referral to the breast clinic for further examination and patient reassurance, perhaps ultrasound if a lump is found? Wonderful video as always, sorry KZbin are being such pains about it.

I’ve a very low threshold for referral, and depending on the history? She’s be referred for further investigation

To reinforce the point even if it ISNT found, the patient has still been able to find it, and thus should be referred There are a few exceptions where you can wait, but I have an exceptionally low threshold for referral to the breast clinic - esp 30+

I think gravity will be an assistant there, but the lump may then covered in greater tissue. I didn’t know this for certain, but there may be the benefit to including both, so briefly assess sat reclined, before then lying flat

If a mass is felt, define the characteristics: Features to note in any "lump" 🚀 (SPACESHIP) 🚀 • Size • Position • Attachment • Consistency / Fluctuation • Edge • Surface and SHape • Inflammation (redness, tenderness, warmth) • Pulsation, Thrill (?vascular structure)

The test is the same, but I wanted to highlight the physical motions that the patient goes through It’s one of the reasons I don’t like the “In a box torsos” Part of the skill of clinical examination is the positioning of the patient, and the confidence of interaction with the patients body. A good example of that here is the way you need to get the patient to relax their arm onto yours in order to examine the axillae
