What kind of cable should I use to connect the bridge 4 to my Hx stomp 5 pin midi input?

A 3.5mm TRS to DIN5 cable adaptor and a din5 cable, or a 3.5mm to DIN5 cable. We sell adapters, and so do many large music gear retailers.

Could this device change the channels on my amp if it's just a mono on/off footswitch? Cheers

Usually amplifiers use higher voltage footswitches which aren't compatible with the BRIDGE's Flexiports by default. There are some devices - usually more modern devices which use 3.3v switching logic and are compatible with the Flexiports, but we created the CLiCK Relay Switcher for just this purpose. piratemidi.com/en-au/products/click-midi-interface-relay-switcher The CLiCK can emulate a FS plugged into your amp, and you can control the relays with MIDI commands from any MIDI controller (hardware - like the BRIDGE, or software - like an app or DAW).
