I dont really understand, did the one laying on the bed pass away. I thought he was in a car accident and survived. And he's rapping the lyrics "I know I'm not here anymore". Im confused.

Yes that is his younger brother the one in the bed he did survive from the accident. They made a video as if he is not here anymore if he did not survive hence why he in in the bed then at the end they show his grave. They made the song as if his little brother was gone that day .Hope that clears up the misunderstanding.
