Turning into a werewolf and starting the quest that gives you the cursed ring of hircine would be a fun wildcard. Its within the rules because its forced on you and the only way to take it off is by it taking itself off therfore as soon as possible is whenever it decides to. The X factor of paranoia due to the inevitable 1 random werewolf transformation could be hilarious. Survival mode would also be an improvement. Another addendum could be that you are only allowed followers that will turn on you and attempt to kill you if you transform into a werewolf in front of another neutral NPC in a hold.
