It is not really possible to show how it works without taking it apart, but a metal star-wheel point rises through the hole in the card which then turns the starwheel so another point on the wheel actually plucks the musical box tooth - just the same as with a disc musical box. You can see starwheels moving on the video of the Capital Cuff Musical Box.

You may make any tune that fits in the tuning scale which is clearly written on the cards, it just takes time and patience.

I bet if you gently pulled the paper through the machine you wouldn't get so much of a rattle from the mechanism. Beautiful playing, well done getting the notes punched out

You can get a quieter mechanism if you buy one with Nylon or plastic gears but the metal ones last a lot longer. The mechanism would need to be screwed down to something substantial to be able to give a gentle pull; but that is certainly a reasonable idea, I will suggest it to the elderly owner.

I would guess that the gear ratio of the crank method versus the gear ratio of “pulling it through would make your suggestion unworkable and likely rip the paper.

No. Not enough range and a lot of Rush E is subdued notes (the weird cross patterns, text, 12 key crashes, and 88 key crashes). In spite of noble attempts… Rush E is literally unplayable on a single instrument.
