Thank you! On a Holter monitor for 48 hours. While moving about, my bottom leads (white, brown, and black) unsnapped. I knew brown was in the middle but couldn't remember the "smoke, fire", etc part. You've saved the day! :-)

EmpoweRN I have been a cardiac nurse for almost 12 years and the cardiac educator for our cardiac step-down unit for 5 years now. I am really not trying to be critical or mean at all but all 5 leads were placed completely incorrectly in this video. This is our policy based on AHA and ACC guidelines. The LA electrode should be placed on the upper left aspect of the chest-shoulder region just below the end of the left collarbone (clavicle). (Much farther up under the end of the clavicle than in the video) The RA electrode should be placed on the upper right aspect of the chest-shoulder region just below the end of the right collarbone (clavicle).(Much farther up under the end of the clavicle than in the video) The LL electrode should be placed in the left abdominal region at the level of the umbilicus. Anatomically this would be on the lower left anterolateral surface of the external oblique muscle. (The ones in the video are much too high on the ribs) The RL electrode should be placed in the right abdominal region at the level of the umbilicus. Anatomically this would be on the lower right anterolateral surface of the external oblique muscle. If you are using V1 for the chest lead, the brown (V) electrode should be placed in the 4th intercostal space to the right of the (patient's) sternal border. You should teach to feel for the intercostal spaces to properly place V1 or any other V lead.

The brown one was not on the heart side, correct? It was her left and not the patients.

I agree. Red on right shoulder, yellow lead left shoulder, green left lower, brown right lower and white on chest (middle).

Amen, it is video's like this that mislead nurses everyday. If you watched this video, you should forget you did. Even an inch of misplacement can cause faulty recordings, therefore faulty readings, therefore faulty responses.

You don't get a reading when green is off because RL is the 'earth' electrode. It doesn't really do much but earth the electrical current, but without it you can't have anything. Interestingly enough it doesn't actually have to be in any position in particular unlike the others, they just teach us that to make the placement more systematic. Pretty cool, huh! I love ECGs. So interesting.

So, I went last week to get my port flushed and I had to take off my leads. Once we finished the flush I had to put the leads on and the nurse was trying to remember what she had learned so she wouldn't forget where to place them and couldn't.. I told her what I had learned in this video and she was impressed. Sorry to bother, but it was totally exciting to remember something my actual nurse had forgotten. Ha!

You have such helpful and inspiring videos :) I'm currently in nursing school, however I've worked as a nurse's assistant in a nursing home for 4 years. Sometimes my patient's family members look down on me and perceive me as inexperienced since I am young (20 years old). I was wondering if you could do a video on how to deal with rude family members, since I am sure this is something nurses have to face as well. Any advice would be greatly appreciated!!

It does matter if the leads are placed correctly & this is a nursing skill & there are specific anatomical locations where the leads are best placed i.e. second intercostal space, mid clavicular line.

Hello. Thank you. I have a holter monitor for 24 hours. The doctor assistant was sick and a secretary the sometimes helps him put me the holter. The back instructions says that the #1 brown was supposed to be under the left chest side on the right position and #2 black next to brown one, but on going the left and down side. She put it on switch positions. And according to the holter monitor instructions these two are not to down from the chest, but these are about 3 inches under the chest. Do you think it will be possible the results will be correct? Thanks

I'm a Registered Nurse on the Med-Surg floor and they're going to have me start training for Telemetry so I found your video to be very informative and easy to understand, it helped take some of the edge off my to be quite frank. I was wondering if by any chance you had a video that talked about some of the important cardiac meds you would be giving to your tele' patients (especially medications for interventional purposes). Or perhaps any additional tips for this branch in Nursing. Any help would be greatly appreciated. Thanks!

Hi , I’m abit confused . I’m not too sure if the camera is reversed. But the placement of the brown lead is more to the right or left ? You said that it’s placed close to the heart but placed on the right , I’m confused 🙈

The way of memorizing the placement is just great, quite useful. :) BTW ive experienced working with monitor which had a yellow plug instead of brown one but its just a detail :) Ive subscribed your channel, i think its worth it. What is the difference between telemetry and medsurg/telemetry? I heard that telemetry was just like a medsurg floor, but everyone has to be on a monitor.

Red ecg lead may be on right arm side ...correct order is ..red, yellow ,green, black (right arm,LA, LL, RL.) Lead V placed on the sternum .. 🕵️

Im interested in the telemetry technician, or ekg/ecg technician ? What are the qualifications and program I need to get my certificate. I am currently working at a hospital now as a housekeeper to get my foot in the door. I am interested in becoming a technician in the cardiac field or start a monitor tech.....any suggestions would be helpful? i am in college also and working full time at hospital

Just In case you are interested, European's including the UK have a different colour code (who knows why). Listed in order of HP / Philips / Aligent 5 lead Cable USA Brown (V), White (RA), Red (LL), Black (RA), Green (RL) UK / Euro White (V), Red (RA), Green (LL), Yellow (RA), Black (RL)

If only you knew why we are so "annoying" you wouldn't feel such disdain. We are usually the first to notice if someone is dying so..

very interesting and def. not universal. Here, in Germany a telemetric ecg has 3 or 4 leads (color coded black, red, yellow and green) and a diagnostic ecg has 10 leads. And I am pretty sure it is like that in most of the other european countries. Why don't they really makes those things universal? We use monitors produced by the same manufacturers all over the world.

So true... a universal method would be great! I have have never seen a yellow one... but we do have 3 lead ecg monitors as well... probably could have mentioned it here... but I forgot lol! Thanks for letting me know!! I look forward to hearing from you again soon!! xoxo - Caroline
