Cardiovascular disease and interstitial lung diseases (ILD)

It is really important to have more awareness of the risks of developing cardiovascular diseases in patients who have interstitial lung disease / pulmonary fibrosis.
Many patients with ILD can develop something called pulmonary hypertension, which is an increased pressure between the right side of the heart and the lungs. This can cause blood oxygen levels to drop significantly and lead to severe breathlessness and other heart complications.
Screening for pulmonary hypertension with an ultrasound scan of the heart (echocardiogram) is really important in most patients who have interstitial lung diseases.
Patients with ILD are also at potentially a higher risk of developing coronary artery disease which increases the risk of heart attacks. Also, they may be at a higher risk of blood clots and pulmonary embolism.
Actively looking for cardiovascular disease is important, as in many cases there are effective medications for many of these conditions. While we may not be able sometimes to completely stop the worsening of the ILD, we may be able to optimize treatments for the heart and vascular diseases, for better long term outcomes and potentially symptom improvement.

I have a CT scan of my lungs twice a year and I have an echocardiogram once a year. I was told I have a very mild case of pulmonary hypertension. Not on medicine for it. I do take OFEV twice a day for upper prodominate lung fibrosis sarcoidosis. My last CT scan was stable. Good information you give to us. Thank you! Btw, my doctor and I suspect mold is the cause of my scarring. I moved to another house and actually I feel much better. My basement would flood at the old house and landlord never fixed this issue and mold was the problem and dampness... I'm in a better home now!

Yeah. We waited until dad was "ready" for transplant... We did heart cath... Showed heart disease. No transplant. He dead. Really sad. Wish we knew before it was too late. Ugh

Dr. Stanel: My friend was given Keytruda as part of a treatment regime for NSCLC. She developed ILD, mainly in one lung. Recently she was given pegfilgrastim (a G-CSF) to correct low blood counts from a chemo drug and the latest CT scan shows ILD throughout her other lung. Main evidence for the ILD is ground glass opacity and need for oxygen (up to 8 Liters / minute - we live at 7000 feet), especially with even mild movement. My question is and a possible subject for a future video: Can drug induced interstitial lung disease lead to hypersensitive interstitial lung disease, or is this more likely to be another instance of drug induced interstitial lung disease. Succinctly: does ILD predispose to further ILD? My friend is waiting for an appointment in June at a hospital that specializes in lung diseases. She is finding your videos very informative.

Very informative thanks. I have ,end stage, IPF and Pulmonary hypertension and am now in palliative care mode. I'm always looking out for self help possibilities and recently read an article about the potential remedial use of Kefir. I had not heard of this product prior to this and maybe you could throw some light on this application. I am on O2 continuously at about 4 Litres/min but any exertion drops my SPO2 to 72 very quick smart. Please keep posting your informative talks.

Yes, I've heard that Kefir helps reduce lung inflammation in animal studies... Also, these medications maybe of some help to you. 1. Saracatinib 2. oral lysophosphatidic acid receptor 1 antagonist 3. Nintedanib (Ofev®) 4. Pirfenidone (Esbriet®) 5. Roflumilast. Hang in there buddy!

@@Unknown-hu4gf Thank you. I was offered these drugs (3 and 4) by my pulmonologist but she suggested that, at my stage, they might cause more harm than do good.

I found a scientific paper where they compared perfenidone with tangeretin, admittedly only in mice that were given a chemo drug that induced pulmonary fibrosis. Tangeretin was more effective in the mouse trial. Tangeretin is a citrus flavonoid derived from the skins of tangerines and mandarins (it is also in the skins of various oranges). The Chinese medicine Chen Pi is essentially sun dried and aged mandarin skins, and is available through Amazon as a tea or just as the skins. My friend has Drug Induced Interstitial Lung Disease with some fibrosis and has ordered some of the tea while she waits for an appointment with a hospital that specializes in ILD. The ILD inducing drugs involved were Keytruda, and recently pegfilgrastim (a G-CSF) prescribed along with chemotherapy for NSCLC. Good luck and thanks for the Kefir suggestion.
