I think I figured out what's going on: This introduces the album. It's the girl finally leaving her abusive husband (and bringing the kids with her). It seems that after she did this, he began following her and attempting to incapacitate her so that he might get revenge. The gash on her skull is explained by the staged car cash--the windshield broke and hit her in the head. She finds herself in the middle of the woods with little memory of what happened, and must find her way out.
