As a retired nephrologist, everytime I come to visit my daughter in school I like to go to her computer and check this videos for fun. The way you conducted the case presentation was excellent starting from the basic tests and building the case from the bottom up. Of course as a nephrologist as soon as I started listening the case the first thing I started to yelled was fluids, fluids, fluids even though I can start the eternal argument with the pulmonologist and the cardiologist but your student can worry about that 10 years from now. Again excellent conduction of the case presentation and very good didactics for your students.

I started as a medical transcriptionist in 1995 playing guess the diagnosis. I later became a nurse. Now I work as a nursing paralegal, but I graduate from law school next May. I love this type of video. I’ve reviewed so many cases where the providers miss the most obvious signs.

I would love a playlist of videos where you concisely review and speak out loud your thought process of several case studies.. like a 40 minute video where you review 30 cases and just speak out loud so we can get your thought process of what is the diagnosis and why you think what you’re thinking so it’s very high yield and concise

Im a med student too, I was literally messed during history taking and diagnosis of diseases.Even though I studied gallon of diseases, I don't know how to apply it when doing a case history and final diagnosis, thank you soo much for this and being a life saver😘❤, pls keep up the good work, if possible pls do case history reveiws on GIT system🥺🥺, loads of love for u and ur team☺🥰😍❤

As a former CNA, UTI is always my first guess with increased mental confusion. It's common. This was fun.

I had a case like this in the ER where 70 yr old female obese brought in from home, pt was mostly bedbound and daughters were caring for her and they report all of a sudden she was non-responsive. The ER attending evaluated her as we did also and he ordered baseline labs and a foley. I did a sternal rub very limited response. Her morbidly obese state interfered with the physical exam. Once we got a foley in her, filled 1 bag and started a 2nd, she regained consciousness and her first question was, "where am I?" The amount of info I learned that day was priceless.

I used their vids during A&P to help me get through tests, Zach has a great way of explaining things that others don't quite have. I loved my instructors, but Zach just has a way about his lecturing that's top notch.

Urine culture, pyelonephritis, UTI, cystitis. Possibly septic w/fever, pubic pain, tachycardia, hypotension. I don’t think it’s SIRS due to localized pelvic pain, color of urine. Pt’s Alzheimer’s dx is also a clue...Alzheimer’s PTs lack hygiene awareness and coupled with the HCTZ could be causing dehydration along with pt. not getting enough fluid intake. P. S. I’m not in Med school or am RN...just an observer and love your videos.

I should add that just knowing the epidemiology of a “shocky” presentation from nursing facilities, esp in women, one has valuable clues about the most probable causes.

1- initially resuscitation with IV fluids 2- meets SiRs and with pos UA -> severe sepsis 2- severe sepsis in setting of hypotension 3- CBC, Cmp, lactate, trop, procal, CXR, ECG, UA with reflex culture, Cardiac monitor 4-insert foley for I&O 5- if BP better admit to medical floor. If refractory Hypotension admit to IMC/ICU for vasopressors 6- empiric antibiotics while in ED with Zosyn and Vancomycin 7- cultures should have been collected prior to antibiotics administration 8- AMS prompts a dry CT head to rule out stroke.

Hi, im a little bit late here. At 31:12, you mention using vasopressin (ADH). Would it really be wise to use ADH in this scenario, when her urine output is already low? Im just not sure if this could worsen her current AKI.

Wish Dr Zach had been my mother's Dr when I took her to the hospital after she was sick and collapsed at home on the 5th day of a 7 day course of Macrobid for a UTI last December,3 days before Christmas. The Dr said her bloods were clear ,and she was just dehydrated,he put her on a drip and said I could then take her home. She collapsed again the next evening as I was walking her to bed, but this time her poor heart couldnt keep going and she died holding my hand. The paramedics tried to revive her but it was too late. Her GP looked at my mother's A&E report from when she was admitted the day before and she said her white blood cell count was slightly elevated and she was surprised they didnt do a Urine test. She had collapsed a month earlier with the same symptoms a d treated for a chest infection and taken to the same hospital,all on record here in London. I am currently going through the hospital trusts complaint procedure.

I recently received their reply,but apart from acknowledging they should have measured her blood pressure standing up before discharging her,they gave me the brush off not dealing with my points. I am still considering if I should take it further with No win No fee medical negligence lawyers.

Hello Zack! amazing video and very interesting case One thing I would like to highlight...I work as an ED/Intensive care nurse in the UK and in a case of septic shock with no response in fluid resuscitation our first drug of choice is usually metaraminol, an α1 agonist causing vasoconstriction to support the BP Is that a drug used in the US or you normally move straight to noradrenaline if you need pressors? Thanks in advance

Zach, c-diff isn't a consideration with those meds? I'm just learning through experience about c-diff, I'm not fully certain which meds cause it, I know Augmentin is a big culprit (happened to me 2x from that med).

C diff is pretty common with antibiotic use in general, some more than others. Also patient didn’t have any diarrhea or constipation. However, always keep c diff in the differential when a person has a super high white count with recent Abx or immunosuppressive condition with an unexplained source of infection.

Possible cause could be Citrobacter Koseri...symptoms are in line with this species of bacteria.
