Case study 125 - Sacroiliac Joint Pain
Sacroiliitis is a painful condition that affects one or both sacroiliac joints. These joints sit where the lower spine and pelvis meet. Sacroiliitis can cause pain and stiffness in the buttocks or lower back, and the pain might go down one or both legs. Standing or sitting for a long time or climbing stairs can make the pain worse.
Sacroiliitis can be hard to diagnose. It can be mistaken for other causes of low back pain. It's been linked to a group of diseases that cause inflammatory arthritis of the spine. Treatment might involve physical therapy and medicines.
The pain of sacroiliitis most often occurs in the buttocks and lower back. It also can affect the legs, groin and even the feet. The pain can improve with movement. (Source: mayoclinic.org)

Thank you for this video. I am a nurse practitioner primarily in urology but work ED as well. I follow your videos and I see this a lot in my office. Many of these patients have indeed been blown off by their PC provider after negative imaging. I will certainly point people to this video for more information!

Thanks for referring to Physical Therapy. Speaking as a PT we are also healthcare professionals who believe in patient centered treatment. We can provide pain relief as well as help an individual return to the desired level of function. We provide education about the dysfunction as well as a home program for self-management.

Thanks. That's the pain I am suffering from right now. I know what caused it, and have been going to the Chiropractor. He never mentioned it what it is.

Thank you for covering this! This woman is basically me. I have other nerve symptoms too, but I hope this is a major piece of the puzzle to a good portion of my pain. My physical therapist thinks it’s this, so we’re trying exercises suited for this.

Thank you so much for this. It’s made me cry. I have serious SI pain on both sides. It’s been about 10 years since it started following meningitis and a prolonged recovery (subsequently ending up with fibromyalgia ) Done all the physio, had Radiofrequency ablation and steroid/lidocaine injections. Radiofrequency last time didn’t work at all. I have pain just laying in bed and end up with my knees bent up to try stop the pain. After seeing this I’m going to try find a doctor who specialises and speak to my GP for a referral. Thank you for making me feel like I’m not going crazy.

Thank you! I suffer from SI joint pain and it is not take very seriously. I see a chiropractor for regular adjustments.

I had what I now know to be an SI joint injury after a weightlifting injury when I was 29. Was blown off by doctor and told that I was just “getting older” and these things would start to happen and unfortunately just took him at his word and assumed 30 really was “where things start to go downhill.” I dealt with 4.5 years of intermittent constant (loud) popping in the joint, pain when walking up stairs and standing still, intermittent sciatica, and pain that was so bad that I struggled to get out of bed in the morning. I also had to give up weightlifting completely. At one point I felt like the only thing that was holding my lower body together was compression leggings (which I had started to wear under everything, everyday for this reason). I do not know how or why (other than the fact that I stopped sleeping on my stomach) but about 6-9 months ago, the pain decreased dramatically and the constant loud popping of the joint when I would rotate my body stopped. Now that I know that pain can remit and it’s not something I have to deal with for the rest of my life, I am willing to go on a medical world tour and pay out of pocket to rectify it if it ever returns.

I was diagnosed with endometriosis and my drs and family at first treated me like I was pretending. This happened for 8 years until my diagnosis.

Excellent explanation! I did have this during and after pregnancy, 44 yrs ago. Now in my early 70's ... I have tried desperately to get a real dx and rx for nearly 9 years to resolve pain in my outer hip, deep into my buttock, electric shocks that start in my buttock and run down my leg and into the sole of my foot and then the fire starts. I've been dx'd with piriformis syndrome ... PT, injections, hot/cold, medications ... nothing sends it packing. The gabapentin does help with the red hot electric shock shooting down my leg, although it does still appear, esp. at night when I am at rest watching TV, reading, etc. Some of those injections were done aided with fluoroscopy in the O.R. I still am in agony often. Another physician I am going to now has ordered an MRI ~ which is scheduled 2 days from now. She thinks I definitely have bursitis and while the pirifiromis is involved in her opinion, she doesn't think I have true Piriformis Syndrome. I can't wait to get this MRI done and see her in 2 days after that for (hopefully) real answers. My PT didn't think it was true P.S., either. I'm about outta my mind with this thing! It really is putting a damper on my ADL!

I got relief with Botox injections on my piriformis muscle. First relief from pain in that area in years. It worked as well as migraine injections.

My right SI joint has fused on its own. Sometimes it feels like bones are literally poking out my booty cheeks. To get an idea what im trying to describe, Google patrick Star bones poking out of butt. This is what it feels like. I can't get my doctors to understand. I've had imaging done only showing that my SI joint has fused (non- surgically) and that I have degenerative disc disease. The fatigue. The aches in random places in my body. The pain. Sometimes it's all too much.

Had coworkers speak to me about awful pain, tests are all negative. So….I began to ask….have you ever had an epidural? "No, just two….". Or…"no, but I had a saddle block". Think of it….a 20 gauge needle. Can it still be called a needle? Think. A layer of bone created to heal…a patch mechanism. Now, here comes aging and thinning bones, also a mechanism. If a sheet of drywall the fastening screw just popped back.

I’m curious about those who experience this pain but have never been pregnant- I do have the anatomy to be pregnant but I haven’t been but I do have pain in the exact area.

Large mal firmed tumors inside my sciatica nerve ..spread up my spine into left breast and lung terminal since 2010 is when found them
