Dean IS affected my Famine but not in hunger of more but in hunger for nothing. He is used to excess, a lot of food, a lot of alcohol, a lot of suffering, a lot of pain, a lot of this and that. He is not hungry (the hamburger) anymore because that's his famine. Yes he is empty and all of that but that's exactly what his craving is, it's nothing, emptiness.

You bite on camera, then spit it all out as soon as you can, and the spitting out is cut out during editing and the finished product comes out with the cutting back and forth between the actor eating and someone else, as seen here.

And you have to do the same for the coverage of the other actors. If you actually eat everything you're filmed eating, you'd either get intensely sick, or intensely fat, or both.
