Hello Oleksii, Thank you for another informative video. I was wondering if you have any more information about the new J&J PureSee? Maybe a defocus graph that compares the J&J PureSee and the Alcon Vivity? I am particularly interested to see how the contrast sensitivity is between them and how it compares to the Eyhance? I currently have a J&J Eyhance in my right eye but unfortunately have a refractive error limiting my distance vision, so looking at options to fix and also an iol for my left eye. Thank you Mark.

Hi Oleksii, thanks for your videos. I just had my 1st cataract surgery (right - dominant eye) on Jan 10 wth J&J Eyhance iol. But not doing well so far. I am short sighted wth power 350 on left eye n abt 425 or 475 on the right. On the 2nd night after surgery I started seeing rather long slanted streak of light across light source on the road eg street lamps, car headlights, yellow bulb light. My doc said it is due to my weak cornea (he said I have Fuch's cornea dystrophy). I went to another eye doc for a 2nd opinion and he said he can see folds on the bag covering/holding the iol (he said the bag 'shrinks' to cover the iol and folds can occur during this process) ... and if the folds do not straightened out, he can rectify this using laser and he said I do not have Fuchs cornea disorder! I am going to see a 3rd doc to reconfirm whether I have Fuchs or not. But my main questions to you (hope to get your input) are the following - the 2nd doc said he had done many cases using Eyhance but the overall outcome is quite unpredictable (some patients do well, but some dont). Thus, he had switched to using iol Lentis (edof from Teleon) which has a much better ... more stable outcome/results. Thus, I am in a dilema now as I do not want to do my 2nd eye under my 1st doc, but unsure of Lentis iol, but my vision has not improved so far since surgery wth Eyhance (intermediate is not quite clear, near range is definitely out). My 1st doc has given -50 for my right eye...when I see my face on the mirror (close range), I definitely cant see the fine details eg. to even out my sunscreen after applying, nor see/pick up fallen hair that stick to my face. My 1st doc said he can give me a mini monovision by giving me options of -100 or -150 for the left eye (to ease my concerns not seeing finer details of my face on the mirror) but thinks I cant adjust to -150. And the 2nd doc does not give any mini monovision as he believes it is not good for patients due to the imbalanced power. Thus, I wld like your advice/input on Eyhance vs edof Lentis (Teleon). Fyi - my left eye macular retina has a hole and right eye has a membrane...thus, multifocal is out. Thanks in advance!

Hi! I'll try to help with the IOL selection. Eyhance is a good and stable lens, however as any lens it needs personalized adjustment at surgeon side and with Eyhance there is a special procedure needed which doctors are not always follow, that's why refractive outcomes are not stable in certain cases. Regarding the Lentis from Teleon. Depending on the Lentis model we may discuss, but as you have a macular hole I'd think about another Eyhance. If you are willing to have better near, I'd suggest to go with Eyhance and minimonovision, but this is a defenitely an option where you shall be ready that images in both eyes will be different. As you had about 1D difference (I assume that the power you mentioned is your pre-cataract refraction) I do not think you will have a problems, but better to discuss with the surgeon why he think you will not tolerate this. Lentis comfort might be a good option but still do not expect a good near vision as the lens is not designed for that, and macular hole is not good condition as well. so I think is better to discuss the Eyhance with monovision options. If possible (if your cataract on left eye is not yet too dense) you may try a monovision with contact lens and see what to expect, at least at far distance. Hope this helps.

The Lentis model suggested by the second doc is Lentis Comfort, he said from his experience more of his patients have better outcome for near vision compared to Eyhance which he had done/used a lot previously. thanks for words of appreciation! yes, that's correct as the purpose o the Enhance as other advanced monofocals is to give improved intermediate, while purpose of extended range of vision IOLs which are EDOF like Lentis Somfort, Tecnis Symfony, Alcon Vivity etc is to mitigate presbyopia and thus it gives better intermediate and a bit of near depending on the model. However again, we have to balance between range of vision quality of the light transmission and aberrations and cost. Wish you to take your best decision with your surgeon!

I'm interested to know what vision I would experience in between the 3 focal points(the defocus)of say a Panoptix IOL. Is there a way to represent this in a vision exam or with spectacles. Thanks

I am on the younger side, have cataracts in both eyes, but one is much worse. I am considering getting clareon multifocal in bad eye and waiting on advances in technology for the other. What is your view on this strategy and expected advances in accommodating and other iols in the coming few years

What is the best lenses that fit or suits for dislocated lens? Because my eldest son needs to undergo eye surgery becuase of his both lens are dislocated. He diagnos with marfan syndrome which his eyes was affected. Now he is 6 years old and we are still waiting for his schesule for operation or surgery. 😢

sorry to hear that. I do not have expertise in such cases. Normally if an IOL needs extra fixation there are surgical techniques used to use suture, or sometimes PMMA (rigid) lend is used with PMMA haptics. I suggest to discuss with your doctor, and most probably it will be a monofocal lens.
