Thing is that both Star Platinum and Jotaro have lightning fast reflexes so the moment Jotaro feels slight pain in the back of his head he will have star platinum stop the pen while its still penetrating the skin. Yes Star is defiantly fast enough to do that.

No, the entire point of the "I am wood, stupid" scene is to showcase that Hazamada does not get damaged when Surface does. He punches it, and he doesn't receive a punch in return. Surface is one of several exceptions.

Stands that possess physical objects ( Like Yukako’s hair and Hazamada’s doll ) don’t reflect the damage back to the user. So, if Yukako’s hair is cut, she won’t die. In the same vein, destroying Surface wouldn’t kill Hazamada.

I think it's because it won't do anything due to how the puppet kind of gives him super human strength like when he hit Koichi so hard he both hit him at a far distance and also broke a door without his stand so his crazy diamond probably wouldn't be able to stop it, but who knows. I think it's due to certain range due to how he was able to use it when he wasn't in range but like I said who knows
