You try to adjust the transverse ridges or inclines, not the direct cusp tip. If you flatten a cusp tip, it widens the contact and you can end up with eccentric interferes on inclines. For 1 veneer provisional, do not use the adhesive. Rely on interproximal mechanical retention to hold the provisional veneer in place. Be sure the bisacrylate goes completely through the interproximal space. Watch the DentistryMasterClasses.com video on fabrication of a single provisional veneer.

A single veneer provisional is held in place by mechanical retention and can be removed with a scaler. Multiple connected veneer provisionals must be cut inter proximally because they are cemented with adhesive (unfilled resin).

You are going to lute them back together once you have adjusted them, so you intentionally cut through the interproximal contact with a two sided diamond disc.

Not wet, just damp is fine, especially if dentin is exposed. Dry is fine if just enamel. Dry is actually fine for all the teeth with provisional veneers because we are cementing the provisionals with adhesive only, no primer.

I always divide veneer temps into parts when there are 8 -10 adjacent veneers in the maxillary arch. If you don't, the distal provisional veneers with "spring outward," like a starched shirt, and it will be harder to seat them. I want the veneer provisionals to seat passively and that occurs when you divide multiple adjacent veneer provisionals into 3 parts. It is simple to "lute" the provisional parts together with flowable composite.

I believe I cover temp. veneer removal in the video. Separate the veneers with a small mosquito diamond, then use a plastic instrument or elevator to fracture them apart and they remove easily. Yes, you can place a small dot of etch on the teeth for added retention.

It takes a morning, from 8-12 or 1 to prepare, provisionalize, impress and take records on 8-10 teeth for veneers. I schedule another morning from 8-12 or 1 to seat 8-10 veneers and reline and seat a nightguard. It takes about 3-4 weeks for the lab tech. to fabricate the veneer case, so the patient is in provisionals for 3-4 weeks.

What do you think about using the CEREC machine to make the veneers in the office?

Not good if you are interested in a high level of aesthetics. They will all be monochromatic. One of the things a master ceramist does is create surface texture and translucency melded with opaque to make the veneers/restored teeth appear natural. I call it a "natural light" appearance if we are restoring all the anterior and bicuspid teeth with veneers and can select the shade. Patients, especially women, in the US want "white" teeth. The shade of veneered teeth has become an accessory, much like red lipstick, blue eye liner, blond hair with purple streaks, etc. If I am restoring all the anterior and bicuspid teeth for a woman patient, the shade is "natural light," similar to the color of snow, but with surface texture for light refraction and some translucency to make them look like natural bleached teeth. It took my ceramic technician and me 10 years to work out this particular shade and overall veneer. Another reason I am not a fan of Lumineers.

In the video at 2:05 Dr. Cutbirth states that, “Because the technician (incorrectly) did not wax a thickness at the gingival line…, I’m going to have to add some flowable composite at the gingival line.” In other words, the thickness of the temporary veneers was too thin at the gingival line so additional flowable composite had to be added.

Yes, you probably will have to replace them unless the provisionals fracture cleanly when they are removed with the elevator. If they do fracture cleanly and not break up, you could probably replace them and lute the separate parts together with flowable composite.

Wouldnt the technician be able to create a bis-acriylte provisional on the model, instead of waxing the prepared teeth? It would save us time and its easier to do it in lab instead of the patient's mouth. We would just have to cement.

Probably, but there has to be some teeth preparation, so you would have to do more than "just cement." I have done it this way for so long and it works so well I have not looked for a different method. I know some dentists try to simulate the veneer teeth prep, or have the lab simulate the veneer teeth prep, of the stone teeth on a pre-op model and construct the provisionals on that prepped model. I tried it once and it did not work very well for me because the simulated stone model teeth preps were not the same as the actual natural teeth preps. I have a funny personality. I like predictability so I can anticipate how much time a procedure will take. With the method I presented, I know quite precisely how long the entire procedure will take so I am not trying to "rig" something that has not worded well.

Thank you for the detailed reply. I thought the model the lab had in your video was already an impression taken from the prepared teeth of the patient. Then I failed to ask what I wanted to ask in the first place. Let me rephrase that. It's going to look like nit-picking, but would we able to do the procedure in this order? 1)Prepare the teeth 2)Take an impression and send it to the lab 3)The patient has to wait either a couple of hours if he/she is free, or has to come back tomorrow. 4)Lab makes a model and created provisionals 5)We cement the provisionals. The only downside looks like that the patient has to wait 2-24 hours if I'm not missing anything. Would it hurt, that much? Thanks again for your attention.

I have never thought of doing it that way. What is the reason you would do it that way? Those teeth would be sensitive during the time the lab was fabricating the provisional restorations plus you would probably have to reanesthetize the patient. The provisional method I am presenting is quite simple and works very well.

It probably takes 30 minutes of my time to complete 10 porcelain veneer provisionals once I have removed the set bisacrylate in the matrix from the patient's mouth. Total time fabricating the PVS matrix on the waxed up model and other miscellaneous procedures, including my time of trimming and luting the provisionals to the teeth, is about 1 hour. My lab tech does the wax up, so I am not counting his time. I am shooting for "perfect," or as perfect as it can be done, with the final restorations. I am shooting for "time efficiency, diagnostic for the laboratory technician and very good aesthetics, comfort and function" with the provisionals.
