I had two implants placed four months ago with an internal sinus lift. The implants protrude into the sinus by around 2 mm. I have been dealing with sinus pain since the surgery, but the oral surgeon doesn't know why. The ENT doctor thinks that the pain could be due to the protruding implants.

Good tip. Just had the experience of seating #18 with tight access and little visibility. Took my hand away and the crown was rotated 180 degrees. Gave credit to oral surgeon for his abilities to place implant in perfect orientation.

The healing abutment is a 3mm, narrow emergence, yellow implant (3.5 platform) Biohorizons... so most likely Biohorizons.

I have just had this done, they numbed but not sedated, overall the procedure was not painful but gums are sore after local had worn off and for about 24 hours after. I've had dry socket the pain was nowhere near the sledgehammer to the face of a dry socket type pain.

I had a dentist or I mean orthadondist who extraxed a heavily murcury filled tooth cracked severely. Whole tooth removal. I do not trust him when he said there could be no root canal done on it because the murcury fill was so deep and the crack also too bad. My other matching filled teeth where done root canal style with no pulling of teeth. So this dentist goes to pull the tooth and it will not come out. I am trying to tell him you are tearing the jaw tissue and breaking my jaw at the joints. He relaxes a bit and than goes to work on it all again. Escalated until I feel tearing and feels like the jaw is being broken and the tooth will not budge. Went through this for 30 minutes before I get pissed and tell him loudly you are breaking my jaw! The idiot who is suppose to know what to do ask me a sedated Not dentist person what he is suppose to do. I respond I dont know but you are breaking my jaw. It took him a hour and he did not ask for more exrays to see what the problem was. He finally gets the tooth out and it has 4 roots that was why no matter which way he pulled they would play off each other and not move the tooth. I was extremely pissed because you can remove a 4 root tooth a different way and not just pull it out and he did none of that. Put me through horrific pain and stressed me to the point that now I fear dentist and I never feared a dentist or process ever before. I will never trust a dendist ever again!

Excuse me that is an old school of dentistry. Now Just a conservative RCT *without violating the working length except for initial 15k file for drainage *will get the job done, by the way i'm an endodontist and i have treated so many cases like this.

Topical as well as local nesthesia is incredibly important for deep cleanings. I cannot imagine performing a "deep cleaning" without administering local anesthesia. If you are cleaning into the sulcus with ultrasonics, scalers, etc., especially if there are periodontal pockets, how could it possibly be performed without major pain to the patient without local anesthesia? That is why I have videos on administration of "painless and profound local anesthesia." I recommend sedation for deep cleaning of an entire dentition just because administration of full mouth local anesthesia is mentally stressful for most patients. Take your practice to the Top Tier. I use tap water mixed with mouthwash in a Scutan plastic syringe with the sharp part of the tip cut off to irrigate the mandibular sockets beginning 7 days post op. I just had 2 implants placed today. Both perforated the sinus. 😢 I don't know how far. The dentist finished the procedure and wants to wait and see how healing progresses. He said one was just a pinpoint.

Perforation of the sinus with an implant, according to all studies I have read and my personal experience, does not pose a problem. The issue is most of the implant must be in the bone to be stabilized. The part of the implant that is in the sinus is not stabilized by the bone, so you do not want too much of the implant in the sinus because that part is not stabilized by bone.

I use 2 power loops because 4 power hurts my eyes, focusing in and out of magnification. My vision has not been negatively affected from using these loops for almost 40 years. Correct. I never place a provisional restoration on the implant/implant abutment at the appointment the implant is placed because I want the implant to osseointegrate for 3 months before biting/occlusal pressure is placed on the implant so the implant is not loosened from biting pressure on the provisional restoration prior to osseointegration of the implant. I'm not sure, I send patients to another office for panos and cone beams. Economy of space and $ in my office. I don't take enough panos and cone beams to justify the office space or financial outlay to own them alone. It's like a vacation home hours away or a plane ride away from where you live. When I was in my 30s a financial planner told me my wife and I would come out far ahead if we rented the finest hotel or house when we took a vacation vs. owning a distant vacation home when you account for taxes, insurance, maintenance and emotional energy spent. Plus you feel like you need to go to that vacation home every time you take a vacation. I feel the same way about large ticket items in my practice I do not use all time. An Endodontist and oral surgeon near my office have cone beams and a near by orthodontist has a pano. Take your practice to the Top Tier. After seeing this hats off to my dentist doing my extraction today. Mine were very complexed like this case. He was very gentle and compassionate aswell as my anxiety was hitting the roof. Not only am I going to be pain free, I left with breathing exercise tips 😂There’s so much involved than just ‘drilling’

Are you restoring the dentition in centric relation, i.e. you don't want a CR-CO slide? Are the wax ups you are fabricating in CR? You did not mention centric relation in the full mouth reconstruction. Thank you.

I did not rewatch the video, but absolutely the wax up for a full mouth reconstruction would be done in CR. Restoring into CR vs. Centric Occlusion is a philosophical issue if you are not restoring multiple teeth. If the patient has a CR-CO slide and they are asymptomatic, i.e., no TMJ pop and click, no limitation of mouth opening, no deviation of the mandible upon mouth opening, no facial muscle pain, etc., do you equilibrate the patient into CRO? Ideally, the answer is yes, but we all do a crown or 2 or some other restoration without equilibrating the patient into CRO. That is one of the many reasons I perform a comprehensive exam and separate consultation of 99% of my new patients prior to "definitive dental treatment" except for emergency treatment. Remember, if you discuss something with a patient ahead of the procedure, and have it written down and explained prior to the procedure, it is a reason. If the patient has a problem, such as developing facial pain and/or disfunction after the procedure, and you did not discuss in words and writing that possibility prior to the procedure, it is an excuse. If a tooth is abscessed, I perform endodontics. If the apical lesion is large, I may perform an apicoectomy with retrofill at the same time as the endo. procedure. If the lesion is small, the endo. alone should be sufficient. If an endo. procedure has failed, resulting in an apical lesion post endo., then you could redo the endo or perform an apicoectomy with retrofill. If the original endo. looks good, then I prefer to go with the apicoectomy vs. redoing the endo. Be sure to discuss the options with the patient so they understand the rational for whatever procedure you perform. Take your practice to the Top Tier. This RD method does not perfectly "seal" the tooth. The rational for this RD method is multi fold, as I explained. Dentists do not utilize the RD routinely because it is a pain to place and the interproximal rubber strips get in the way if you are preparing a tooth for a crown or any interproximal restoration. This method is terrific because it takes about 15 seconds to place, keeps 99% of the water out of the patient's mouth because of the isolation and you have an assistant aspirating, it prevents large objects like pieces of old filling or crowns being cut off out of the patient's mouth and it keeps the tongue, lips and cheeks out of the operating field. I often use this RD method for extractions because it is so simple and 99% effective. Bottom line, dentists will use the RD using this method. They do not routinely use the conventional RD because it is too much trouble. Take your practice to the Top Tier.
