Yes. We will be doing videos on all areas of dentistry. The intent is to make multiple videos of all procedures so you can see the procedure performed in many different cases, many different situations. For those of you interested in comprehensive, complex aesthetic and restorative complete cases we will have those available in September 2017. Those cases will go from beginning to end and be longer than the current dental minute cases.

How long will the implants last and how long will the new teeth you put on the implants last?

The implant and crown could last indefinitely so long as you do not smoke or eat hard foods and you have good home care and visit the dentist regularly for cleanings.

@@centerforard how do I go about finding a dentist that will perform this procedure. So far I’ve called 3 dentist in my area and none of them do this. They all tried to talk me into another bridge.

Probably because they do not place implants. If the dentist does not place implants, you may need to find a restorative dentist who works with an oral surgeon or periodontist.

Ha, ha! It takes time and effort! I have a videographer who videos my cases each morning. Almost every morning I see one patient from 8 - 1 or 2 o'clock (5-6 hours). The cases may be full mouth reconstructions with vertical dimension increase, porcelain veneers, implants, crown and bridges, periodontal crown lengthening or grafting, full mouth extractions followed by small diameter implant placement with bone grafting and PRF or other procedures. Every hour of video requires at least 4 hours of editing. Once the videos are edited, I am filmed describing the procedures on the videos. My procedure descriptions are filmed almost every Friday and Saturday morning from 8 - 11 or 12 o'clock. We then go to a wonderful barbecue restaurant in Waco for lunch (Uncle Dan's Barbecue). Friday and Saturday afternoons, when we are in town, I either play golf or mountain bike ride some incredible trails on our ranch outside Waco with Bx, our chief-of-staff and CEO of our teaching center (CARD), along with one or two other serious riders. Life is about balance. We work hard but also take time for play and leisure. That is important! Get in the habit of not worrying about things. Things usually work out and it does no good to worry! Plus worrying dentists make their spouses and children crazy and tense! A sense of humor and learning how to relax and enjoy life goes a long way! Play with gusto! Take vacations. Don't worry about the money. Most spouses would rather be with a broke spouse with a sense of humor who enjoyed their life and did not worry about things than a rich spouse who was always worried!

Thank you for the wonderful presentation. But can’t it be worrying if we punctured the sinus membrane? How would you know if the sinus membrane got fractured and how you you manage ?

I do not know of any other systems that have depth limiting shoulders on their drills. I find it hard to believe all implant systems do not have depth limiting shoulders on their drills. I have discussed this drill system with several implant companies and none of them is willing to change from drills with black lines or scored lines, which are very imprecise and difficult to see when the drill is spinning covered with water and/or blood. It makes no sense to me why all implant drills would not have depth limiting shoulders. Going too deep, especially in the posterior mandible, is the greatest worry with implant placement. I recently got a bridge to replace 2 missing teeth on my right side. I had to sacrifice my healthy canine and back molar a abutments. The final bridge has been cemented but I’m not in love with it. I realize now I should have just put 2 implants in. Dentist told me that down the line i can just cut the bridge and leave the abutment crowns on and implant the 2 missing teeth. I feel like an idiot for going the bridge route. Do you have experience with someone replacing a bridge with implants, because they changed their minds?

In my opinion, it's not as predictable without reflecting a flap because you cannot tell the condition of the alveolar crest, i.e., is it flat or is bone grafting needed because of a defect.

Sir I have a doubt...in the initial radiograph length from the crest of alveolar ridge to the floor of the sinus was 10.7 mm apporox....but when you did the pilot drilling your took the safe length as 12mm...won’t it perforate the sinus? Sir can you plz explain this method of taking approx length for the pilot drilling

I prefer to slightly perforate the floor of the sinus because that engages the dense bone of the floor of the sinus, which makes the implant more secure.

An implant or a fixed bridge would be the first choices. A removable partial or "Nesbitt" removable appliance would be another option.

Yes, I place implants regularly in patients with medical problems. A patient taking bisphosphonates is an exception. Read the article in DentistryMasterClasses.com library on treatment of patients taking bisphosphonates.

Dr. Cutbirth, a problem you mentioned with surgical guides is that it is more difficult to place accurately after the flap has been reflected. Could this be solved by punching a hole in the tissue for a guide, or is reflecting the flap necessary for the procedure? Thanks!

I like to reflect a flap so I can see the architecture of the bone, particularly the alveolar crest. I am using a surgical guide you order and soften with hot water that seems to be excellent. I have made some videos with this guide that will be forthcoming in the DentistryMasterClasses.com library. No problem with flap interference.

Dear Doctor Steven, it's great work and awesome presentation. But I want to ask you why you are doing the incision from the palatinal side? What is about doing the incision from the buccal side? As I know the stitching in the Palatine more difficult than mocuse memberan in the buccal side.

It is harder to reflect and retract the flap to the palatal side. It is much easier for you, the clinician, to retract the flap and hold it to the facial side than to reflect and hold it to the palatal. Reflecting and holding a flap reflected to the palatal is an awkward angle. You would have to have the hand holding the retractor coming in from the opposite side of the patient's mouth, making it awkward for you and your assistant.
