Central Retinal Vein Occlusion (Animation)
Central Retinal Vein Occlusion (CRVO) is a significant eye condition where the central vein in the retina becomes blocked or occluded, leading to impaired venous blood outflow from the retinal tissue. This blockage can cause a sudden or gradual decrease in vision due to the accumulation of blood and fluid in the retina, resulting in swelling and hemorrhages in the area. CRVO is often categorized into two types: ischemic and non-ischemic, with ischemic CRVO being more severe due to the higher risk of neovascularization and subsequent complications such as neovascular glaucoma.
The risk factors for CRVO include hypertension, diabetes mellitus, glaucoma, hypercoagulability disorders, and other systemic conditions that affect vascular health. Symptoms typically involve a painless decrease in vision in one eye, which may be accompanied by the perception of floaters due to the bleeding inside the eye.
Diagnosis is primarily made through a comprehensive eye examination, including fundoscopy, which reveals retinal hemorrhages, dilated tortuous veins, and retinal edema. Additional imaging studies, like fluorescein angiography and optical coherence tomography (OCT), can further assess the extent of blockage and retinal damage.
Treatment focuses on managing the underlying causes and preventing further vision loss. While there's no cure for CRVO, therapies may include intravitreal injections of anti-VEGF agents or corticosteroids to reduce macular edema and improve vision. Laser photocoagulation therapy may be used in ischemic CRVO cases to prevent neovascularization. Patients with CRVO require close monitoring for complications and potential progression from non-ischemic to ischemic type.
CRVO highlights the importance of vascular health and the need for regular eye examinations, especially for individuals with risk factors, to detect and manage such conditions promptly.
