When an acid or base dissolves, it produces H+ or OH-, so it appears on the right side. But here, we're doing some other (completely different) reaction, and it just happens to be in a solution that's already either acidic or basic. The H+'s and OH-'s will appear on whichever side of the equation helps to balance it, because water (and therefore H+ and OH- ions) is basically in unlimited supply.

If you were going to divide, you'd have to do it to ALL of the coefficients, even the ones for ClO3- and I2 and stuff. Before we cancel, it says we have 3 H2Os reacting with 5 ClO3-s and 3 I2s. and we produce 5 Cl-s and 6 IO3-s and 6 H2Os. Rather than saying we put in 3 H2Os and get out 6, we just say we get 3 out (the difference). It's simliar to how the equation 3x+5=6x simplifies to 5=3x. You're just subtracting something (3x) from both sides of the equation.
