Sir,today i have gotten a patient around 50 yrs old male presented with breathing difficulty/shortness of breath with fatigue on walking some distance.There is no history of fever ,pedal edema.H/o similar attack.Releived on injection theophylline.No h/o chronic smoking.No family hostory.On chest x ray=multiple nodular opacity in the right lung.So what may be most likely diagnosis???

Depends upon the size of nodules. If the size of nodules are 2-3 mm, you should think of Miliary tuberculosis, Silicosis. If the size of nodules are larger, think of Metastatic disease. These are just a few most common differentials amongst a large number.
