So the thing living inside of Geto is the same thing that lived inside of the guy who did the stuff to Choso’s mother (the guy in the kamo clan who did things to choso’s mother was taken over by the same thing that took over Geto). It’s been hopping from body to body over the years in order to extend its life. Yuji and Choso ARE connected by blood, choso stated himself that he can feel changes in his blood related siblings. The connection will be explained later in the series, but they are related.

In the introduction of the death paintings we are told that they were an experiment by "the evilest sorcerer in history", Noritoshi Kamo, so yeah you already knew Choso and his brothers had blood powers because they share blood with the Kamo clan. The brain inside Geto is not a curse, it was never stated to be a curse, thats a sorcerer able to switch bodies, and its Choso's father, "Noritoshi Kamo". He has been switching bodies since god knows how long, and "Noritoshi" is just one of his many stolen identities. Choso's blood powers are whats telling him that him and Yuji are related, if theres anyone you can trust about blood relations is the guy that controls blood lol Choso mentions that while he and Yuji were born 150 years apart, that doesnt make it impossible for them to be blood related, considering that Choso's father can switch bodies thus avoid aging.

This section of the story is absolutely worth checking out in the manga for clarification and some added symbolism. In the previous episode the author compares Mahito to a rabbit and Yuji to a white wolf. In the manga, before Fake Geto is shown in the winter metaphorical landscape, a black wolf also appears, symbolizing that more than one predator is after Mahito. "Shall I save you, Mahito?" is actually Fake Geto mocking Mahito, and they both know the score. "I always knew, after all, I was born of humans" is a fitting final line from Mahito before he gets manipulated and turned into Fake Geto's plaything. He is saying he knew all along that Geto would betray him - hence the attempt to kill him in desperation - because he was born from mankind's hatred for itself (betrayal being a big element that leads to hatred). While it is unfortunate that Yuji did not get to put Mahito down, it is quite poetic that a curse whose curse technique was manipulating the soul, is himself subjected to a curse technique that manipulates curses. Also just to add, excellent voicework in this episode from Geto/Fake Geto's voice actor. The character he is playing now is completely different from teen Geto and also crazy Geto. He is more akin to an arrogant lecturing teacher who loves to yap and his voicework does a great job reflecting his personality.

So, Choso has 3 parents, his mother, the cursed spirit that impregnated her, and a third party guy who facilitated the process and mixed his blood into the kids(the death womb paintings) as a process. This man's name is Noritoshi Kamo, and he is from the Kamo clan, the same clan as the student from Kyoto jujutsu high. This is why choso, eso and kechizu have blood based abilities, as one of their parents was a blood manipulation user. However, the real villian is the brain thing who was piloting Noritoshi Kamo's body to do these heinous acts, as if you look at the image of Noritoshi Kamo, you can see scars on his head, just like geto. So what choso is saying is that the brain has existed for a really long time, and could have had other kids(for example, yuji), so he is GUESSING that yuji could be his younger brother as he strongly felt Yuji's death Don't worry, this situation will be explained in Season 3

My favourite detail from this episode is how it completes the comparison its been subtly painting between Mei Mei and Nanami. Nanami: -absolutely hates putting children into battles and tries to avoid it whenever possible. -Quit his job because he valued the good he was doing for people as a sorcerer just slightly more than the money he was making -technique is designed to have him be a frontliner, face to face with the enemy for the best effect Mei Mei: -has no issue using children, and has groomed her younger brother to idolize her and support her in any way she asks. -believes that any person is only worth their monetary value and prioritizes helping people who will pay her or will owe her a favor she can call on later. -technique with her crows is the embodiment of forcing others to sacrifice themselves for her own benefit. Nanami is burnt badly and on the verge of death and still puts what's left of his life on the line to fight transfigured humans while fantasizing about retiring to Malaysia. Mei Mei faces "Geto", but ducks out before taking any serious damage and runs away from not just that battle, but leaves all of Japan to its fate, cashing out her stocks before the Japanese market crashes and hunkering down in, where else, Malaysia.

Like Choso said, with FakeGeto jumping bodies, anything is possible. The theory Choso is working with is that FakeGeto who fathered Choso while in the Noritoshi body (thus giving Choso and his brothers blood manipulation powers) is also one of Yuji's parent during one of his body exchange as well. This might explain why Yuji has such high cursed resistance to Sukuna, or why he is so physically gifted.

So the thing controlling Geto is technically Choso's father. Choso and his brothers are the Death Paintings, which were created when a curse somehow impregnated a human woman, and a nasty Jujutsu Sorcerer took interest and made her do it multiple times more while also infusing his own blood into the mix. The Death Paintings consider him their father. That Sorcerer was Kamo Noritoshi, the worst Sorcerer in history, and ancestor of the student Kamo Noritoshi (confusing I know) who goes to Kyoto. What Choso realised in this episode is that his father, Kamo Noritoshi, wasn't actually a human Sorcerer like everyone thought, but in fact is the same brain thing that is now controlling Geto. The brain just keeps jumping bodies and taking on their identity. Also, the Kamo clan is one of the 3 Major Clans along with the Gojo and Zen'in. Their inherited technique is Blood Manipulation, which explains why the Death Paintings have blood based techniques. Sorry for the essay, but I hope that clarifies things somewhat. In this episode, it was revealed that the brain inside fake geto is that man. He created the cursed womb paintings (choso and his brothers) 150 years ago. Choso, for reasons currently unknown, senses the connection between himself, fake geto, and yugi.

The "curse" inside geto isn't an actual cursed spirit. It is a jujutsu sorcerer who's cursed technique allows him to swap from one body to another by replacing his brain with the body's original brain. He was in kamo noritoshi's body too, which makes him the father of all nine death painting wombs including choso. Kamo noritoshi is a member of the kamo clan (probably the clan head since he had blood manipulation, which choso inherited) and the brain guy was in his body 150 years ago just like how he's in Geto's body right now. He also said "kamo noritoshi is merely one of the names I had" which means he's in fact older than 150 years old, since that was noritoshi's age.

So just to break it down, Choso first feels that he's connected to Yuji by blood, because of his ability. (Its not Yuji's doing, even though a similar thing hapened with todo, that seems to be a coincidence as far as we're aware.) Naturally Choso has an existential crisis and starts putting the pieces together. His father is Kamo Noritoshi, the man that dead the whole create the cursed wombs by getting some pregnant with cursed spirits. Notice that both the kamo he remembers and geto currently have the same stitching, implying the same sorcerer is his father Kamo, or more accurately, whoever who had also body swapped with Kamo. Notice how, when he explains how his power works in how he can feel when his broithers die, he remarks that because Kamo/Geto has been body swapping for centuries its 'not an impossible idea.' This implies that not only Choso and Yuji are actually related, but Kamo/Geto is their mutual parent. REgarding blood manipulation, its a passed down technique in that Clan. Choso and his brothers inherited it in some form.

The brain inside Fake-Geto is the same guy that experimented with Choso's mom, it's shown through that moment of Kamo Noritoshi having the same stitches on his head as Fake-Geto. If you want more clarification, you should rewatch the season 1 finale and then this episode right after each other and focus on everything related to Choso and his brother. Also, it's explained that Choso can sense his brothers through his cursed technique, and when he tried to kill Itadori, he suddenly sensed his impending death with his technique. So it's incredibly likely that Fake-Geto is one of Yuji's parents. (I say incredibly likely because for anime only viewers, they have no other info on Itadori's parents.) That would also explain how Itadori has such physical strength and capabilities. Hope this helps

the brain looking curse inside geto is Kenjaku. He is just another ancient curse from the Heian era. Kenjaku's technique is allows him to transplant his brain into another person's body and take over their body. Most people mistakely think that Mahito tried to betray Geto in that scene but it was the other way around. From the start Geto's plan was to absorb Mahito and he knew this so he tried to kill Geto beofre he got the chance to kill him. We can see it was his plan from the beggining because in the Mechamaru vs Mahito, when Mahito was almost dying Geto said something along the lines of "This might be the moment I get-" saying that if Mahito would lose he would absorb him before Mechamaru could kill him

Can’t tell you who Geto is (possible spoiler) but he kind of gives it away in this episode, “that’s one of the many names I’ve had” (implying he had many more and is likely far older than that). The name that was famously pointed out is Noritoshi Kamo (coincidence that the first name of the student from Kyoto also has the same first name). The stain on the Kamo family Noritoshi Kamo of the past seems to be similar to Geto (a curse inhabited his body and not created by the original Noritoshi Kamo) and conducted extremely vile experiments. If you remember in season one the cursed womb paintings were Noritoshi Kamo’s science experiment on Choso’s mother. If Choso is saying Yuji is his brother, then they are blood related or Yuji may be a cursed womb painting as well. The answer will be revealed in the future.

So, no spoilers explanation : Kamo Noritoshi (fake Geto) isn't a curse, he's a sorcerer who has a technique that lets him switch bodies. Choso then mentioned that his mother got impregnated by a curse because of Fake Geto, and then Fake Geto also mixed his own blood into the pregnancy. meaning Choso has 3 parents, a human mother, a curse, and a sorcerer. then, he explains his technique lets him know with absolute certainty when one of his blood-related siblings is going to die. That exact effect triggered when he was going for the killing blow on Itadori. It's why he was tweaking earlier, he was in shock over having tried to kill his little brother.
