Thank you so much for this wonderful video ❗️WHY DON’T DOCTORS KNOW ANY OF THIS? I was diagnosed with RA in 1995 and Fybromyalgia in 1998. No one ever told me that Fybro causes sleep disorders and memory problems. I have been to sleep clinics for my terrible sleep, neurologists for my problems concentrating and remembering, psychologist for depression. I can’t drive at night because the headlights are blinding. I have a super sense of smell ( useful sometimes). I have been in constant pain since 1990. Opioids and NSAIDs only help a little. Now it all makes sense.

They also fail to disclose the correlation of IPS and the EBV. 98% of chronic pain patients who develop Intractable Pain Syndrome have positive Epstein-Barr Virus IGG and/or IGM titers. Rheumatologists, neuros, and radiologists commonly deny this all-too-common explanation. They'd be jobless if they admitted EBV is causing most of the issues of their rheumatoid arthritis and lupus cases. more info to come...

No one told you because no one knows. If you live with an invisible chronic illness and you don't get lucky enough to find a doctor who wants to do the digging and figure out what's wrong, you will spend years trying to find one that does or figure it out yourself.

"Tender Points are stupid." That was hilarious! And I totally agree! The first doctor who "tested" me for fibro - i.e poked me in a bunch of spots with her finger, one of which I said felt good and asked if she could rub a knot out for me - said, "Well, no I guess it's not Fibromyalgia. Maybe it's time for you to accept this as your life and get a rocking chair." I was 32. My husband at the time wanted to punch her in the face. Some doctors don't realize, understand, or care that things that they say hold a lot of weight and can stick with you...there's a reason I still think about that. I was depressed for weeks after that appointment.

I disagree on one point. Heat such as heating pads help me a lot. Many with fibro have neck pain, for instance. I am having a flare right now and heat is helping as much as Tylenol. He likely doesn’t know how heat can take away horrible menstrual cramps… I used to immerse myself in hot baths!! It would help when I couldn’t take codeine for instance, the medication made me very sick.

One symptom of Fibro is IBS which gets much worse when taking painkillers tablets, so tablets are not something that resolve the problem. And in a body where the dopamine levels are imbalanced the painkillers actually do not work, it is the same as taking a sugar pill - it doesn't work!

Heat helps me personally. I do understand that it is a personal thing. Cold actually either triggers the pain or makes it worse. Heat, or even just putting a jacket or blanket on, can help immensely. At times, a heated blanket is my friend. But everyone's body is different. There are those, I would not doubt that neither heat or ice would help.

Thank you so much for this Dr Clauw. In the UK the GPs don't seem to give me any help for my fibromyalgia apart from medication. I'm hoping to try some of the other things you have suggested, but my problem is that last year I broke my leg in 4 places and had to have reconstructive surgery - which still hurts and has limited my mobility. Have you any suggestions for something that I can do with this limitation - I'm pretty disabled by this, and find that my mobility is very limited, as is my movement - my knee is still very painful over a year after the surgery and limits me so much and I have to use crutches to walk.. Can I have the address of the Pain Guide website you recommended? Many thanks for any help you can give.

I have one question or perhaps it is observation. If opioids don’t help with fibromyalgia pain, then in your study where more opioids were needed post surgery for the fibromyalgia patients to control the post surgical nociceptive pain, then the outcome of the study is that fibromyalgia patients need “more” opioids to manage the nociceptive post surgical pain. The opioids don’t turn down the amplifier, they reduce the level of the nociceptive input to the amplifier. I don’t know how they do this, but in my 20 years of chronic pain this has been my experience. I have a severely arthritic cervico-thoracic spine. It gets inflamed often. Extra opioids at those times are a merciful option in my case. But now I am restricted by the VA to a ceiling Rx that means I will suffer if I “overdo” my neck motions. So then, please give adequate amounts of opioids to these post surgical patients instead of rationing them. And people who exacerbate their nociceptively painful tissues by overuse and have a turned up amplifier need these opioids available also. This can prevent a weeks long pain flare up in these type patients. And Yes! Walking is so very helpful. But pacing is crucial, and it takes time and setbacks happen. But the benefits are fantastic.

Yes Fibromyalgia can cause tinnitus in one or both ears. FM can cause many strange symptoms other then pain and fatigue. Most FM patients have sleep disorders and not just insomnia but rather we cannot achieve Slow Wave Delta Sleep or Restorative Sleep as a result of Alpha Wave intrusions. Slow was delta is when the body falls into deep sleep or REM. This deep Delta is when your body heals and restores your energy for the next day. Our bodies cannot achieve this stage of sleep and this is where the vicious cycle of FM begins. We have severe fatigue and not just the feeling of being tired but complete exhaustion as if we have been sedated and become bedridden. Keep an open mind that CFS Chronic Fatigue Syndrome (a.k.a.) ME is not always a symptom of every patient with FM. Substance P in Central nervous system wreaks havoc on the body and can cause a multitude of symptoms but not everyone with FM suffers from all the same symptoms as the next person. The pain pathway in the sympathetic nervous system is disrupted by signals which originate from the neurotransmitters in the brain. It's as if our brain cannot shut down like the volume on music blasting when the concert is suppose to be over yet the band plays on.

My family doc, pain clinic doc, and neurologist obfuscate neuropathic pain and nociplastic pain so it’s frustrating as a patient to not get a diagnosis but a vague categorization. My pain doc rightfully seems most clues in as he said I likely have central sensitization which I now know ‘is’ nociplastic pain. No major acute pain even preceded my pain, except for a lifelong battle with anxiety disorders and depression. I’m assuming that having a genetic component (being female, having and aunt with fibro/widespread pain) means I likely have nociplastic pain. I just want to hear from my doctor a more succinct diagnosis than “neuropathic pain” as it doesn’t fit.
