I know this video is a bit old, but you weren't singled out for the covid questionnaire! Currently, Japanese residents have to make a day-specific reservation to get into Puroland. Tourists can't make a reservation, so instead they can purchase advance tickets for any day, but have to answer the seperate covid questionnaire.
