I listened to 18 hours of immunology lectures you provided on KZbin!!! Thank you so much for making this information available to me! When I taught about how to manage the side effects I focused on the mechanism of action because it makes so much sense. It made an impact and drove the point home that effects from immunotherapy looks similar to regular “ Chemotherapy “ but has to be treated very differently. Many doctors treated inflammation in the lungs as if it was infected with a microorganism, when the etiology was the drug causing the immune system attacking the lungs. My 49 yo sister died of colorectal cancer, almost exactly 7 years to the day from when it was discovered, already at stage 4. She was a hero though for what she endured along the way. At the end, when she had exhausted all chemo avenues, they 'allowed' her into a study for immunotherapy. That was a big mistake. The cancer, which should not have ever entered her brain, went there so quickly that they took out a walnut sized tumor only to have it return within a month! She died shortly after that. I'm not trying to put down immunotherapy (big improvement from chemo, that's for sure). It's just that I am mad that they never told her that she was just a guinea pig to see how much of a dosage they could use on patients. They knew very well what this would do to her. I hope these new studies produce continued good results. Colorectal cancer is a horrible thing.

I was interested in this video because one of my neighbors told me that eating fiber decreases risk of colon cancer. I do not believe that fiber improves health, but I don't have enough education to understand. What I do believe with confidence is that cancers grow using fermentation of sugar.
@dr.akshayprakash57359 ай бұрын
Fiber does improve health. It bulks the stool, it gives more satiety after meals, its food for some of the good gut bacteria and many more effects. Low fiber diets have been linked with increased rates of colorectal cancer. Your gut bacteria controls your immune system, blood sugar metabolism and even how you feel. Having more of bad gut bacteria can devastate your health. So keeping the good gut bacteria thriving with the right diet is important.
@blahblah67259 ай бұрын
@@dr.akshayprakash5735 I just don't believe you, doctor. 1. "It bulks the stool". I don't want bulky stool. 2. "It gives more satiety after meals" - only if you eat carbs. Nothing is more satisfying than meat. 3. "It's food for some of the good gut bacteria" - which produces gas. Saying the bacteria is "good" is a value judgement, not a factual statement. That's the language of bias. 4. "Your gut bacteria controls your immune system" - no, its the other way around. Your immune system controls your gut bacteria. Along with fiber comes toxins that allow gut bacteria to penetrate the gut wall. 5. "blood sugar metabolism" - Animal fat is best for blood sugar metabolism. Animal fat has not glucose, and does not promote insulin response. 6. "even how you feel". How you feel is improved by vitamin B, by ketones, by magnesium and iodine. By avoiding insulin resistance in the brain. Not fiber. 7. "Having more of bad gut bacteria can devastate your health" - you are implying that meat causes bad gut bacteria. It does not. 8. "Low fiber diets have been linked with increased rates of colorectal cancer" - only in demographic observational studies which confound consumers of ultra-processed food with consumers of carnivore diet. I can believe cheerios are low fiber and the carbs promote cancer. Not meat. Amazing how your one paragraph comment about fiber can spout that many canards. Re-iterating my original point. Cancers grow by fermentation of sugar (glucose) in the human body. Fiber almost always comes in foods which increase blood serum glucose. Many fiber foods also bring toxins that increase gut permeability. Listen Dr. Been. I am not against you. I loved your video about Banting Lecture 2011: Hyperinsulinemia: Cause or Consequence?, Barbara E. Corkey, PMID 22187369 . That was a masterpiece.

Then start at 5pm on Friday & take mostly nonstop calls from 3-4 hospitals & some pt. calls. That continues until you are back in the office on Monday Morning. Not an easy job. And I used to do WE call for hospice. On call pay is lousy, you do it because you care. The problem is that our entire society has gone mostly Godless & Self centered.

Dear Dr Been, thank you for the talk. I'm sorry for tempering you enthusiasm but: - it's not a scoop that anti-PD1 work very well in dMMR cancers, especially colorectal. Similar results are seen for already a few years with other anti-PD1 drugs (such as Pembrolizumab) in CRC. It had to be confirmed in dMMR rectal cancer, and particularly evaluate if it allows de-escalating the rest of the treatment (surgery, radiation etc.). However the company needs to do some promotion (with help of the NEJM) and make it appear as revolutionnary because it increases instantaneously its share value. - So yet another "me too" anti-PD1 from GSK. You know how RD works now with big pharma, they copy each other and then there are no comparative study to assess if anti-PD1 X is better than antiPD1 Y in a given cancer because all clinical trials are funded by these firms. The challenge is then to arrive first in some indication where the other firms have no approval yet for their similar product. So here we have localized dMMR rectal cancer. - There are often HUGE discrepencies between phase 1 studies and the rest of drug evaluation. - Unfortunately only a minority of rectal cancers (and other types) are dMMR, and so, likely to benefit from this kind of drug as monotherapy. But we are working hard to improve the response rates, by combining immunotherapies etc.. Kind regards Dr Pourmir, MD

"wishing it had been found earlier"...fully agree....then why are colonoscopies on a every-10-year screening period as to allowable Medicare coverage? Criminal....in my view, in that there is greater chance of first indications found after Stage II or III cancer.....when it could be better-handled after Stage 1, if done more frequently. The US medical "system" is criminal and broken......as we saw the last 3 years under the "you-know-what" worldwide scare...and good meds like I-V-T were downplayed and ridiculed.

I'd like to have a better explanation about things. I came to USA in my early 40s in my country I just knew about 2 cancer cases before that on people that I saw face to face. A friend's friend and my best female friend. It was a colon cancer and doctors gave her 2 months to live she did not accept chemo or radiotherapy, she went in a fast for 2 weeks and stop eating cooked food after that just salads and fruits for 2 months. 26 years later she is alive and thriving she is 72 now and very healthy. When I came to USA in my first job here there were 6 people with cancer at the time I was amazed. Everywhere I went I saw people looking sick everywhere mostly very obese people. I could help some people at my work offering them advice on simply remedies. (not the Americans ones they looked at doctors like gods that know it all, big mistake) You see cancer is a metabolic disease if you change your lifestyle you can cure you cancer. Go on a keto diet or carnivore one stop eating out just cook your own food, exercise or keep your body moving all day, get a balance life looking for peace of mind, escape the drama in your life and you will be healthy.
