Thank you for such a wonderful explanation. I was practicing this skill in my nursing class and doing it wrong, but there is no explanation in my book on why you have to do the test with the stimulus half-way between you and your patient. Your explanation helped me understand this and will help me do it correctly in the future.

Awesome explanation Sharon! One of the things I have techs ask all the time about this is how to record abnormalities in a specific quadrant in the PATIENT point of view versus OUR (Examiner) point of view since the docs seem to always want it pathological. I would love to have you explain this as you may do a much better job than me!

The standard in ophthalmology, John is to document from the patient's point of view as this is how automated VFs are recorded. That being said, if your physician wants it from the examiner's point of view that's what you must do HOWEVER it should be documented as such ... "documented from the examiner's point of view" so when your records go out to other practitioners it is clear how it was documented. Another method is to simply note "defect is in supero-nasal quadrant of VF OD"

John Pignone Reed yes, VFs are documented from the patient’s point of view; not the examiner’s point of view. This is how automated VF print out are oriented and is the standard way to document defects. An easy way to flip it around from your view to the patient’s view is to document the defect on a scrap piece of paper as you see it, then FLIP THE PAPER OVER, hold the paper up to a light so you can see what you drew and draw the defect as it looks like from the back of the paper. Alternatively, the examiner can document the defect by naming the quad ie “constriction in superonasal quadrant OD”
