One of my pupils isn't perfectly round. Something I just noticed. Also have loss of balance and dizziness/headaches often. Getting my eyes checked in a couple weeks because not sure what to think of it all 🤷♀️

Thank you for posting this!!! Surprisingly a lot of nurses assess the eyes differently and there is some discrepancy on how to properly do this neuro exam! If there is only one option for pupil size, is it more appropriate to record and report pupil size with room light or direct light with pen light? Thanks!

Typically we report the size pre light, and then you will note they were reactive and equal to light in your notes

@@NurseMinder we denote deformities and damage of the pupil separately. The PEARL is the basic assessment, but the roundness and and pupil damage is addressed depending if ocular damage has occurred or if it’s a genetic defect we review and assess it independently as the prime purpose of pearl is a base line.

When do we measure the size of pupil when it dilates after constriction?? Do we place that strip of measurement in front of pupil??

Yes you can do that. Over time you will get comfortable just looking at them and sizing them

I'm so frustrated with accommodation in charting. For 1 it's locked in with all the others as an all or none and 2 I can say it's charted more than 99% of the tjme without ever being checked. And worst of ALL is 90%+ of the nurses don't know what it really is.
