RECOVERY trial closes recruitment to convalescent plasma treatment for patients hospitalised with COVID-19
15 January 2021
www.recoverytrial.net/news/st...
RECOVERY trial is being conducted by the registered clinical trials units with the Nuffield Department of Population Health in partnership with the Nuffield Department of Medicine
176 hospitals across the whole of the UK
Since May 2020, randomised comparison of convalescent plasma vs. usual care alone
Convalescent plasma has been widely used as a treatment for COVID-19 but
To date there has been no convincing evidence of the effect of convalescent plasma on clinical outcomes in patients admitted to hospital with COVID-19
On the advice of the Data Monitoring Committee (DMC), recruitment to the convalescent plasma arm of the RECOVERY trial has now closed
No convincing evidence that further recruitment would provide conclusive proof of worthwhile mortality benefit
Either overall or in any pre-specified subgroup
N = 10,406 randomised patients
Deaths, 1,873
No significant difference in the primary endpoint of 28-day mortality
Convalescent plasma group (Experimental group)
18%
Usual care alone group (Control group)
18%
Risk ratio 1.04 (p = 0.34)
Follow-up of patients is ongoing
Final results will be published ASAP
Recruitment to all other treatment arms - tocilizumab, aspirin, colchicine, and Regeneron’s antibody cocktail - continues as planned.
Professor Martin Landray, (Oxford)
Once again, the RECOVERY trial is demonstrating the value of large randomised trials to properly assess the role of potential treatments.
Professor Peter Horby
This is the largest ever trial of convalescent plasma and it was only possible thanks to the generous donation of plasma by recovered patients
and the willingness of current patients to contribute to advancing medical care.
We owe them all a great debt of gratitude
Whilst the overall result is negative, we need to await the full results before we can understand whether convalescent plasma has any role in particular patient subgroups
Dr Gail Miflin
We await the full results with interest, there is no doubt that this trial will provide a real answer to an important question
NHSBT has helped deliver the largest ever randomised control trial of convalescent plasma
The story so far
www.recoverytrial.net/news
RECOVERY trial finds no benefit from azithromycin in patients hospitalised with COVID-19
14th December 2020
Lopinavir-ritonavir is not an effective treatment for patients hospitalised with COVID-19
5th October 2020
No clinical benefit from use of lopinavir-ritonavir in hospitalised COVID-19 patients studied in RECOVERY
29th June 2020
Low-cost dexamethasone reduces death by up to one third in hospitalised patients with severe respiratory complications of COVID-19
16th June 2020
No clinical benefit from use of hydroxychloroquine in hospitalised patients with COVID-19
5th June 2020
Current RECOVERY trial arms
Low-dose Dexamethasone (now only recruiting children)
Colchicine (commonly used anti-inflammatory)
Tocilizumab (an anti-inflammatory treatment given by injection)
Convalescent plasma (collected from donors who have recovered from COVID-19 and contains antibodies against the SARS-CoV-2 virus)
Regeneron’s antibody cocktail (a combination of monoclonal antibodies directed against coronavirus)
Aspirin (commonly used to thin the blood).

I'm an ICU nurse in the US, we witnessed the uselessness of the covolescent blood transfusion. Almost every single patient ended up on a ventilator and died, but there was no eureka moments at all that we found what helps to get a hold of covid. Unfortunately it takes trials and time to find out what is helpful and what is not. It would of been very good.

That's one of the advantages of the NHS; its joined up. Speciality staff will be on first name terms with counterparts all over the UK. Setting up trials, (whilst acknowledging its difficult), is nonetheless easier because of the system. A national communique can be sent to all NHS staff via email. City hospitals will have very good communications with local GPs etc. It allows for a co-ordinated & reasonably quick approach particularly in an emergency, but also for mass recruitment for volunteers. And if advertising is needed; well the BBC will get that done, on TV/Radio/iPlayer etc, nationally, tout de suite...😉

Compare efficacy of 0 to 30 % of dexamethasone and others interventions from Recovery study - with efficacy from calcifediol (active form of vitamin D) as covid treatment. Calcifediol has 2500% efficacy as shown by Castillo et al. And do not listen to people who say this trial was too small. I studied stats and can tell you that you only need 1000s when you are looking for 30% improvement. For 25 x improvement 30 patients is fine and Absolutely proves treatment works. They had 76. People who say Castillo randomised control led clinical study was too small simply do not understand stats. We are not alarmed or worried because these are very rare occurrences and they occurred in very frail patients with very serious disease,” he emphasised. “We are now asking for doctors to continue with the vaccination, but to carry out extra evaluation of very sick people whose underlying condition might be aggravated by it.” This evaluation includes discussing the risks and benefits of vaccination with the patient and their families to decide whether or not vaccination is the best course".

I'm in Florida, had a regular appt with my primary care doc yesterday, he puts me in a moderate-higher risk category, tells me I am looking at mid-March for vaccination. However, if they start seeing an unexpected dramatic increase in case positivity, then he would put in a recommendation that I am moved to a higher priority group. Numbers have remained relatively flat here over the past few weeks, so hopefully I stay on track for March. Florida tends to get "poo-poo'd" in the more left leaning press, but if one looks at the actual data from our Department of Health, one will see the state has actually done a pretty good job of balancing spread of the virus, especially in light of keeping the economy fully open. Also, the same press tends to paint Florida as a place where people are ignoring masks, social distancing, etc. Living here, I can tell you that this is simply NOT true. You don't see most communities and businesses being overly pushy about enforcing these things, but for the most part people are doing these things on their own. You actually see far more non-compliance here in the cases where a city or county being heavy handed in enforcement. Overall people here are responsible unless local governments try to overstep their enforcement rights. I think the media is more or less taking political shots at our Governor for not being heavy handed. The truth is that Florida is what is called a home rule state, which means most legal authority devolves to local governments and the state government and Governor has limited rights for statewide mandates. My personal opinion is that regardless of politics, the Governor has done a decent job of balancing this, and I think the actual data bears this out. I always advocate for looking less at what a government leader says and look more at what they are actually doing.

I’m in Pinellas county and was able to get an appt for next week. I was texted by public health asking if I was interested. Upon my affirmative response they said to wait for the call. Appt within a week. I suspect this contact was due to earlier testing at the local public health dept. So check with your local department. In Florida you don’t need any other qualification other than age. Good luck to you, my friend.

I'm in California and agree. Mandates never go very well and haven't here either. If those in the know would educate, if the news showed what it's really like from day 1, there's far less anxiety because people know or at least have an idea what they are dealing with, and can act accordingly. Hospitals here in S. California have no ICU beds and are diverting. We had field hospitals when we didn't need them and don't have but a few now that we are really needing them. Top-down management doesn't work well, especially when not based on science, yet even if it did, scientists disagree and are sometimes wrong. I know things are going to become far worse here in the next few weeks. It's bizarre to have people lining up for food lines. Volunteers get name, address & number of people in a family from the car window, you drive into a line where eventually there's a sign to have you "Pop your trunk" where other volunteers load food into your car, then close the trunk. There's no human to human contact, just food in bags placed in your trunk. It's so bizarre to see so many cars lined up into makeshift lanes in a church parking lot. So many cars being loaded with food basics. It's very surreal. Most people work from home who can, many have lost jobs, many businesses closed. More people raising chickens than ever. It's so strange. We are just coming into what's going to be a really bad time period here. Cases and deaths are going up. They say the case doubling rate is still around 29 days, yet I believe it's more like 25... in any case, that would mean our hospitals are going to be twice over capacity within a month. I heard a report about the governor here ordering body bags and refrigerator trucks. It didn't sit well with me and actually I was kinda pissed. I know people are going to die from this virus, but the mandates, the backlash, the protesting... WTH for? It's going to spread, people are going to die. I've known and said that from the beginning. Was all the theatrics really necessary? It seems to me that as far as numbers go, we might be better off if there had been no mandates (& no groups pushing racist/anti-racism dichotomies in the news media.) For years we've known and seen what mandates do where there are outbreaks. There's always fear and always an angry backlash, then it gets worse. Perhaps this was necessary to get the vaccines done so fast? I've seen them take many years. There was just as much craziness, debate, politics and nastiness surrounding treatments and such, only crammed into a much smaller time period. That's certainly saved lives. What an interesting time we live in. Thanks for sharing what's going on there on the right coast. When things settle down from all this, will people learn from this? Will policies be changed? Or will there be those who feel the need to tell others what to do, even if they are wrong? We thought 2020 was a year we would rather not have had... I hope the whining, screaming and growth pains are over because there's not going to be room for it here soon. People need to be responsible for themselves. I hope that's a lesson people learn from this. We're going to loose people here. I've already told a few at my church quite a while ago to expect to know someone that's going to die from this. Many now already do. We're hearing more and more from people we know that have it. I expect it won't be long until we know of a death locally, then more. Perhaps that will give some people some perspective? I dunno, we went through Hong Kong flu and TB in 1969 and people didn't lock down here. People became more careful. We're living in some interestimg times. I hope things continue to go well in Florida.

Regarding recovery trial. I note that in the Recovery trial the patients receiving the convalescent plasma were already in hospital AND doing poorly. That might indicate the patients are already in the inflammatory phase of COVID illness. I also note the monoclonal antibody therapy is being tried on covid positive patients early on in their illness before they become poorly. I suggest the subset of patients to have a trial of convalescent plasma should be those in early phase of illness when such antibodies as neutralizing antibodies before they are poorly as the plasma might then make a difference.

That's why we have been saying you must treat early, with Ivermectin even the UK e-bmc evidence based medicine consultancy which advises the Uk health department and the WHO has come out and said Ivermectin must be used immediately in the UK. 49 Ivermectin studies 18 of them peer reviewed, But hey he is more interested in the doom & gloom that you people feed off............

I agree. I'm all for listening to children's explanations at this point. Give them a basic problem of how this bugger works, give it to them in toy form (say sponges and water... something non-threatening, just play) and see what they come up with... Children look at things differently than we do... maybe facilitate a "Happy accident" thought process that would lead to a therapeutic.

In the years to come, tens of thousands of people with recognize good Dr. Campbell for summarizing this scientific research for the common man. No govt. may recognize this, he might not receive knighthood or be decorated with medals, but we the people, will remember the calming voice of reason, of science and hope. Thnx doc, in times of need, you've been the difference between panic / depression or getting overwhelmed by this crisis and hope for the future.

I used to do plasma donations every 3 weeks. 40 minutes 830 ml . Fortunately the pandemic paused that effort since I am not going to go sit as an asthmatic in a group of potential spreaders without mask. But once I get my shot and my wife gets a shot, I will continue again.

@@Jake-dg3vt A lot of stuff is filtered out when doing plasma donations. Plasma donation is mostly done for antibodies and other stuff that's in the plasma. Heh, antibodies for the Shingles... nice, I had the singles, but no way am I going to donate plasma. Anyway: plasma donation after vaccinations is pretty normal. But it is wise to check before going. I don't see a specifiek yes/no listed, so I would have to call.

My mother got a text about plasma donation 2 weeks after she had died, something of a shock (she didn't have a smart phone so my phone was used). But no matter, the science is important even, or especially, when it shows that something doesn't work. There was a ton of stress around the circumstances of her death, she caught covid in hospital after being taken there to be checked after a fall, and then when she returned home (thank heavens), a gp said she should have another test - which came back positive. However everyone I spoke to on the end of the Covid helplines was so helpful and concerned, it helped enormously. Apparently another test after so short a time would very likely be a false positive. She came home and passed away peacefully with me there.
