FOR DIFFERENT TYPES OF GUIDEANCE - PRIMARY CODES FOR EACH WITH MODIFIER 59 ON SECOND CODE FOR EG : TWO FNA , ONE WITH MR GUIDEANCE , ONE WITH CT GUIDEANCE - 10011 ( PRIMARY CODE FOR MR GUIDANCE ) AND 11009-59 ( PRIMARY CODE FOR CT GUIDANCE ) SO MAM'M MY QUESTION IS WHY IS THE CT GUIDANCE HAS A MODIFIER 59 ON IT , HOW WILL WE KNOW WHICH ON WILL THE PRIMARY CODE AND WILL ONE WILL BE THE SECONDARY CODE WITH A MODIFIER ON IT

See the guidelines for FNA. "When more than one FNA biopsy is performed on separate lesions, same session, same day, using different imaging modalities, report the corresponding primary code with modifier 59 for each additional imaging modality..." The guidelines don't state which should be considered the primary one, so that is up to you. Maybe use the bigger or complex lesions.
