When I started playing, one of the first decks I made was a MB LD with a similar idea in Eventide Standard. Rain of Tears, Drain the Well and Poison the Well until you do a Nightmare Incursion or two and exile all lands from your opponent's deck.

Surgical on lands is only correct against degenerate lands: Tron, Cloudpost, Saga, etc. Surgical on that Island in Round 4 was incorrect. Against aggro-control mirrors, the Surgicals should have all been boarded out. The reason is that Surgical is good when there are only 2 card names of an effect you wish to dismantle--that's when Surgical is best. Against Murktide, Troll, Bowmasters, BrazenB, Reanimate, Grief...you can't possible cut their creatures off. You don't care about their lands. And you don't care about disrupting their permission. So no Surgical at all in the deck for such matchups.

In other words, Surgical is fundamentally about critically reducing the structural stability of the opponent's deck. If their deck is too redundant, that plan won't work and it's time to pivot.
