1:39 red chests pull from the red chest pool, but whenever you roll an item normally (d6, perthro etc) you roll that item into the item pool of the room you are standing in. You were in a normal room, which corresponds to the treasure room pool, meaning the jacobs ladder came from the treasure room pool.
