If there's an intensivist who doesn't value his/her nurses, then they're in the wrong business. We're at the bedside a fraction of the time while they're there THE WHOLE DAY.

Eddie, as far as I know, this is the *one and ONLY* video of its kind on KZbin (a day in the life of an intensivist). As a recent college grad aspiring to become a critical care physican, your channel is an invaluable resource. While there's plenty of FOAM material on critical care out there (I'm a longtime emcrit fan), there's pretty much *nothing* like your channel on KZbin. I can't thank you enough for these videos!

Welcome to the CVICU! I love caring for those pts. It's a lot of fun resuscitating those pts after CABG, taking care of impella's, all the pressors fighting vasoplegia/cardiogenic shock, fluids, extubating. Glad you found your way onto my channel. Hopefully I'll present you with some more stuff that will be helpful along the way.

very honest perspective. This is exactly how a day is for me as an intern. It sucks to get pimped on rounds but he is right on being thorough because people are veryyy sick. Also Eddie please make a video on Mechanical Ventilation/Vent Settings !

Wow, I am so glad I found your channel. I am wrapping up my 2nd year as pre med. I am a tech in the ICU at my local hospital and I love it. I know working in the ICU is what I want to do. Wow! You mentioned PA's and I just thank you for that. I graduate in 11 days from PA school. I actually start my final rotation tomorrow (yes, I only get a week thanks to COVID) in the PICU. I will be following a PA and I know everywhere differs but I wanted to get an idea of what exactly goes on. Thank you! Also, my fiances sister is actually an ICU travel nurse. :)

I am trying to figure out how to not only be a good nurse for my ICU patients but how to be efficient for my intensivist. But as I see how attentive you are on your rounds I feel like my docs, one in particular is just an ass. lol Keep up the good work. Id be thrilled to work with someone so dedicated to the job.

Diego, it seems as if we all meet somewhere in the middle as Intensivists. Most of us just dive head first into our day as opposed to other specialties where I've seen meetings and administrative duties occupy the first couple hours of the day. Glad to see I'm not totally insane with how I work then! Cheers!

I agree. I watch everything on KZbin at 1.5x. I can’t even stand to listen to myself when I edit these videos bc it’s so slow. I leave them at their recorded speed most of the time bc KZbin pays more for videos >10 minutes and also because much of my audience is outside the US and have commented that I speak too fast.

Finally a doctor who appreciates his nursing team. Sad the attitudes and abuse physicians have towards us. Was NICU nurse for 10 years and NNP for 8 years finally had enough.

Hello! Thank you for your advices. I am 6th year medical student and i consider Intensive care as my future speciality. The other one is cardiology. I have a very mathematic thinking in general and I would particulary like to have the clinical and interventional part in my daily job. Night shifts and extra working hours is not something i can't handle. Which do you consider that are the best things that happen in this speciality as a daily basis? Thank you!

This is a great video and I love how you take notice and reply to everyone's comments. I'm waiting for the upcoming match, and have a strong interest in Critical Care. So much that I've been doing research in Critical Care, trying to get as many papers out that I can before starting residency. How important do you feel having research papers in credible Critical Care journals is towards getting a prestigious Critical Care fellowship?

Thanks, Hassan. Hopefully you're going to match into a shop where they have a strong Pulm/Crit program or Critical Care program. Like that they'll get to know you quite well during the course of your residency where you absolutely need to kick some serious butt. Make them NEED to take you as a fellow. In my opinion, that's much more important than having papers published. I have one paper published. One. I got into the fellowship by carrying a reputation of working hard and having good bedside manner. The place where I did my residency didn't have an in-house PCCM nor CCM program so I had to go elsewhere. In my case, and I will say I'm more of the exception rather than the rule, it would've been much better to have those papers under my belt. If I was a program director looking for candidates for my Critical Care fellowship, having those published papers would definitely be attractive to me, but you need to have the clinical acumen underneath your belt, too. If I were you, I'd keep working towards more publications, not just because they're helping you get in somewhere, but honestly our field needs more people doing that. There's a ton of work to be done with regards to trials and other studies. Best of luck in the match! It's less than a month away. Let me know where you get in.

@@eddyjoemd Wow, I don't know how I missed this response. I actually did match into a program with a great pulmonary/critical care fellowship program in-house (NewYork-Presbyterian Queens). I've already spoken to the group and made my interest known. I will be working in the MICU for the first time next week and I'm really excited! I see what you mean. I actually heard that it's harder in general to get into a CCM only program compared to PCCM, is that true? Did you do a medicine residency or something else? Because I've also heard that medicine residents are best suited for PCCM over CCM only, and that anesthesia/surgery residents are better suited towards CCM given the higher exposure to the SICU and neuro ICU. What do you think?

Thanks for this information doc. I am in my 6th year post-medical school in Nigeria, been into GP private practice. I feel very drawn to critical care and all high acuity stuff...But I'm torn between Critical care medicine and critical care surgery. I know its kind of my decision to make, but any advice for me? hi, thanks for the video but i want to know if possible for intensivist work in other places not icu only such internal medicine or anywhere, outpatient clinc .

I'm not saying it can't be done, but it would be quite difficult to master and you would have to jump through numerous hoops with regards to finding a hospital system/group (if you're in the US) to hire you to do all those things. Ultimately, I don't think I'm sufficiently intelligent to master those three disciplines to the level where I'd like to be for one. You also need to think about call schedules and patients calling you from your outpatient clinic with backache while you're taking care of a critically ill patient who is actively crashing in your ICU. It's quite difficult, at least in my opinion. Hopefully you can pull it off, though.

How is compensation for intensivist? Is it less comparatively? I am a resident in pulmonary and critical care medicine, I am always disheartened when I hear that we are paid very less. And intensivist life is very hectic

This is unacceptable. I recommend this book for basic management of the vent. Whenever you respond to a code blue outside the ICU, how do you set yourself up/get everyone to follow your lead and what information (if any) do you try to gather to help resuscitate the patient? I am also a med student, have been in a few code situations and still don't feel like I could ever lead a code like I've seen even interns do.

+folumb practice. Whenever I get to a room where people don't know me (which rarely happens anymore) I take command of the room by politely but assertively introducing myself and getting a quick update on the patient. While someone is talking I'm looking at the patient and doing my CAB for ACLS. You don't need the whole scoop in that first minute. Just get compressions rolling and the rest will fall into place.

Hi Eddy, im a Med Intern very interested in critical care and i really liked this video! I have a question for you, How does a Critical Care physician schedule work? Do you have 12 hours day shifts everyday? How often do you have night shifts? Thank you for your videos again!

It varies but I have a 7on-7off type of schedule. 20-30% of my shifts are at night. Best of luck!

I didn't answer the other parts of your post but my days are 10 hour day shifts and 14 hour night shifts. It could get longer as that is just the nature of the beast. That's the way that they had been doing it in my group bc the 10 hour shifts make it easy to take kids to school and to be able to eat dinner at home with ones family.

Isn't intensivists doctors are anesthetists as well? I wonder, because in Europe the whole specialty is called Anesthesiology and Critical Care Doctor

In the US, the short answer is no. I can't speak for other countries, though. You can be an anesthesiologist and an intensivist with an extra year of training. You could also be an internist and then an intensivist with additional training. Does that answer your question?

Hello Dr. Joe, Thank you for great advice. I just matched into a community program, but I would still like to match into critical care. What advice would you give me to turn by next few years into the best preparatory years?

I also did my residency at a community program. Best thing ever for me to not have fellows around. It allowed me to grow exponentially. Try to do as many ICU Rotations as possible and then some. Try to do an away rotation where you would like to do your fellowship. I had a ton of intubations, lines, and other procedures under my belt when I completed my training. It also allowed me to work closely with the ED docs and they would call me for their procedures.

@@eddyjoemd Thank you for replying to me. Is there a great emphasis on research for fellowships, or should the main focus always be, be really good clinically. I enjoyed watching a few of your videos. For someone interested in Critical care: your channel is gold!

You should absolutely try to get some sort of research done in residency. It only helps, especially if it's a ICU type of project. Sometimes the ICU docs are motivated to get some research done but want ambitous interns and residents to carry the studies forward. We all have tons of research ideas in our minds.

Residency is a grind and you're going to be an underdog, as I was, coming out of a community program. Use that opportunity and grit to push harder than anyone else and be the absolute force that you can be. It'll be worthwhile when you get into your fellowship and your attendings are going to celebrate you. Best of luck. Let me know if you need anything else along the way.

Depends on the job. I do about 25% nights.

I.... and I mean with a capital I.... Include your nurses in the plan of care. Listen to their insight, their fears, their questions, and so on. They have a wealth of knowledge and information about the patient. Treat the patient and not the computer. The patient is the family, the RN, the RT, the dietary staff, the wound nurse, and anybody else who has direct care of that patient. Lastly, truer words have never been spoken about concise, subjective, and informational notes. It will save your area and perhaps the patient. RN's dig through those to have a better picture of what's going on. This piggy backs off of including them. It may save you a page later anyway.

Hey Eddy, please make a video on what your intern year was like and how you synced in with the learning curve and honestly what does the intern/residency learning curve looks like. You are very accomplished and fluid with your daily tasks, I am curious to know what the process is like for a clueless med grad who enters a program and climbs the ladder.

Exponential. That's one word I can use to describe the learning curve. The first 6 months are a mind blowing extravaganza of learning, especially if you put your passion into it and have a great attitude about it. I was also clueless, but clueless with work ethic. It started leveling out towards the end of first year when you become a (pretend) know it all. Then you become a second year and it all happens again with the learning curve bc you're going to have some (hopefully) smart interns asking you a million questions which will disappoint you when you don't know the answers. That'll motivate you to study even more bc you're competitive deep inside with yourself. Once you get the hang of it, you'll start working on your flow, your efficiency, your own style. It may not come quickly but once you get there, it'll all fall into place. You'll achieve that fluidity you mentioned that I have midway through your third year of residency. Just enough time to realize you're kinda cocky until you start studying for your boards. Then you'll realize you don't know anything again. Once you take and pass your boards you'll have a good feeling about yourself again, that is, until you get into some issues with patients where you're dumbfounded and start looking around for an attending only to find you're there by yourself. No video necessary. That's residency in a nutshell. Don't forget to eat, sleep, and enjoy yourself along the way.

eddyjoemd Woh now that's the level of honesty I was hoping for, I always knew in the back of my mind that it will be this sort of a self sufficing process , I just wonder why this is not talked about in the mainstream conversation about med residency. Or perhaps we re inventing that conversation right now, the more you dissect and make videos on your experiences.
