Just used this last night on a 1 year old croup patient. It worked like a charm. I remember watching the video when it first came out. Just take (3) 1:1000 1mg/ml ampules and put them in a nebulizer and boom.

Hi, thanks for the comment. Epinephrine affects both alpha and beta receptors therefore you will see the B2 effect for asthmatics. Hope that helps and thanks for watching.

Is one better than the other when considering racemic epi or the straight 3ml 1:1000? And is there any cons for underdosing this? My protocol is for Epi 1:1000 1mg with 3ml NS nebulized. I would imagine this is far less effective.

What about asthmatic patients who are tachycardia? Epinephrine would actually worsens the tachycardia.

Patients can be tachycardic for a number of reasons when are experiencing a severe asthma attack. It could be from dehydration, their underlying illness, or as a result of their respiratory distress and impending failure. In this case the epinephrine has the potential of improving their respiratory distress, enhancing their diastolic blood pressure and therefore reducing their tachycardia. I would never withhold nebulized epinephrine in a child simply because of tachycardia. Great question and thanks for your comment. Tachycardia is not generally a good reason to withhold epi. Most people in shock, or even just sick and stressed about their health, will be tachycardic!

3 mL is for under 1YR, then 5 mL for 1 YR and above.

Does it mean that Racemic Epi (0.5cc +3cc NaCl) hits only b2-adren receptors, while Epi 1:1000 (3ml) activates both b2 as well as alfa-1 receptors? Thank you.

Consider the racemic and epi 1:1,000 the same. They both hit both alpha and beta receptors.
