How to get the Secret Ending Mission: "(Don't Fear) The Reaper"
Steps:
1. Have a generally good relationship with Johnny. To track this it is the middle box labelled "Synergy" with a percentage underneath on the pause screen.
2. Do the Johnny Silverhand Side Missions; "Chippin In", "Blistering Love", "Holdin On", "Second Conflict", and "A Like Supreme"
3. During "Chippin In" follow these specific dialogue options when the time comes:
~Inscribe Johnny's Initials~ "Let's do something about that."
"The Guy who Saved My Life."
"Nah, fucked that up too."
"What do you want from me?"
"OK. But as second chances go, this is your last."
"You were a real dick in the beginning."
"When you said you let down your friends..."
"Smasher biz really got to her."
"Yeah, I'll call Rogue."
4. During "Nocturne OP55N1" choose an ending option with either Panam or Rogue. You can wait at this point or choose "put this to rest" and then wait. Sit there and stare at Johnny, your pills, and the gun for 4-5 minutes. (I cut out about 3.5 minutes of sitting there before Johnny intervened)

You only get the option to call someone if you're in a romantic relationship with them.
