Working button holes (surgeon's cuffs) are a tell-tale sign of a bespoke suit. Here's what you need to know.

Will definitely be implementing this buttonhole style on my suit jacket/blazer! I love the detail of it! Thanks for sharing, Ryan!

To the general public: if you are buying off the rack and need alterations to the sleeves, DO NOT get a jacket with functioning button holes. Your seamstress/tailor will thank you. Your statement about off the rack having fake button holes is incorrect. Unless the jacket is custom, all button holes off the rack SHOULD be fake. Otherwise, how do you hem? Unfortunately, all of the suite jackets that have been brought to me to be hemmed were off the rack and had functioning button holes. No one who buys a suit jacket off the rack wants to pay a fortune to have the sleeve shortened from the shoulder. Not to mention it tightens the sleeve. I wish designers would just leave button holes uncut for mass production. pssst....please pass it on!

Wow, this info is actually very helpful. I almost went on the look out for jackets with working buttons.

I know this is an ancient vdo but I'm looking at my first tailored adult suit and really wanted to learn about working buttons. You said a lot, fiddled a lot but showed practically nothing. Clearly the cameraman hesitated to get anywhere near you and your cuff buttons. WHY...?
