If you choose to accept he will hit you with an attack that is impossible to avoid, and instead of a Gameover screen Sans says "Get dunked on" with some different music in the background. This phrase became iconic in the fandom being the focal point of lots of fanart and memes. This phrase however did not originate in Undertale and is not limited to Undertale, however Undertale is the probably the fandom that uses it most often and has made it an official part of the fandom.
