You really need to get your act together. Quite a few of these were not fails, so your title is a lie. Also, get your terms correct. Several times you called an excavator a dozer and the plane is a "water bomber." You also commented on the operator's "skill" during a fail . . . If the operator really was skilled and experienced, the fails would not have happened.
