Living in la the horn goes off at all times. Cities can apply to be quiet zones but you have to invest in making the train line safer... No city paying for that.

In America they are usually required to blow a crossing cadence with the horn between a whistle board until the engine enters the crossing. Long - Long - Short - Long. FRA rule 14L I believe as a rail fan but not an expert. I believe the bell is used more so when entering station platforms or operating at slow speeds when people on foot are near the trains. Again, not an expert.

bell is used any time it's near people. a horn can be heard for miles, the bell , maybe 100 feet. if you can hear the bell, you're BERY near the train.

trains sound fir crossings always. in fact, they can (and some amtrak engineers do) blow for crossings even if a quiet zone.

Honestly a lot of the missions are really half baked ideas. There's another one on this route that makes no sense. You pick up a lost hiker's wife and you're supposed to go find him. Except you pick her up almost 20 miles away from where she says she lost him while they were hiking together, then the mission is glitched and you cannot finish it. It never acknowledges that you've found him even if you did.
