Why don't countries replace the bridge wing .50 cals with Simbad? A better than Sidewinder 90+% hitrate? It would add another layer of air/missile (and silly little boat) defence, plus even a small ship with 8VLS could have, God knows how many: 70 missiles? An Arleigh Burke could go from 90, to easily over 150, like the"old" Ticonderogas, 'cause they're man portable.

Simple answer: Price vs potential threats, what you suggest for AB-class may cost $1B+ considering how many there are. They are already expensive & elaborate weapons platforms. 50 cal is old but dependable & relatively very cheap, easy to train & supply. It's capable of meeting low-tech attacks in a variety of conditions. Missile replacements cost $1/2-1M each & their launch/fire-controls must be integrated into ships' combat systems. They also add weight topside which must be accounted for to maintain ships' stability, etc.
