Join me for a day in the life of a doctor on call for 24 hours! I'm on call for internal medicine, assessing patients in the emergency department, admitting patients to hospital and covering any strokes.

Doctor, your videos really help me see the continuum of care and what happens after we get them to the hospital. As a paramedic, I have a really limited idea of what happens when they get into the hospital as an admitted patient. I appreciate you showing us what happens. Also going through different problems that patients face is an amazing resource too!

Yup, all of our specialists come down to see admitted patients in the ED, including the intensivist if they are destined for the ICU. Patients get the same care as the floor in the ED, but without the safer patient ratios! I took care of a step-down level patient, a 1:1 ICU patient, and a half-dozen or so medical patients. Our hospitalist, intensivist, and OR team all came to me for report when they came to consult. I love ED--never the same thing twice, and always a chance to learn.

Dr. Siobhan! I'm in medical school and I'm secretly hoping to do residency in internal medicine someday but I feel like I'm always lacking in the "case-solving" aspect of it all, since our exposure to the hospital became limited due to the pandemic. I find myself watching medical shows to practice (although these cases are always exaggerated a bit) so I can relax but review at the same time. Your videos are a creative and new way for me to continue practicing and I just wanna say thank you for putting in the time and effort to put out videos like these. It helps me be inspired but at same time learn while enjoying. Cheers!

I can totally relate ! I've just started working as a neonatologist in a new hospital. It's my first experience as a graduate pediatrician. I've been called to assist the birth of premature babies in the delivery room and OR. Last Friday I had to resus a late preterm baby who was born apnoic with very low heart rate! I had already done that but I was used to had the attending physician by my side in case something went wrong. Now I'm on my own.

Wow you are so smart!!!! I love that you could hear an aortic stenosis through the stethoscope. The doctors I work with aren’t specialized but omg they just use radiology to diagnose everything

@Violin MD Siobhan, I totally understand what you meant by double checking everything ...been there myself. It was so interesting to learn about the TAVR procedure. Poor Mark, he looked so buggered. And the on call bedroom looked very nice. I never could sleep when I was on call as I was always looking at my pager.

My mom had the same issue with her heart valve and just had a TAVR in March and recovered so fast! Ironically a friend of mine now used to work Edward who makes the artificial valve. ❤

Thank you so much for doing this video. So many people my entire life. (born with a vsd and coarctation of aorta) don’t honor or respect my wishes to not lift a heavy object or walk too many stairs etc. (outside a med. setting) They reg. forget that heart issues are not just an “age thing”. And it is not all related to diet or salt. … Like we did it or caused this narrowing or hole. They mean well. By no means are most people purposely non objective. Nor rigid. They just don’t know that it is not as uncommon of a problem as it appears to the outside. ❤️ Triple thanks 🤗 🙏

I hear you. I also have invisible disabilities. Mainly my knees, also the effects of being hypothyroid. How to convince someone I have no energy to do things, rather than being considered lazy. 🙁

Then when we push too hard, and struggle to move or do a daily task, to people please they are like “why didn’t you tell me”? 😂 😑 Like it has not been a thing the entire whole lifespan of our knowing one another. I required aortic valve replacement surgery via Open Heart Surgery last April (2022). My valve was regurgitating. It was bicuspid, but also found out post-surgery that it had a 4-5mm hole in one of its leaflets. Only symptoms I experienced were a low blood pressure of 90/40, and blurry vision in my right eye as if someone was shining a flashlight into it. Recovery from OHS was very tough. My valve was replaced with a bovine valve. It took about a month for my body to “accept” it. My heart wound pound every night when going to sleep. That caused me a lot of anxiety. Luckily at about 1 month post-surgery my heartbeat was routine again. One year later now - I feel much better. My BP is stabilized. I had my surgery in a very busy New Jersey hospital. Nothing like the one your working in now.
