Can't remember if it was the Gold or Silver age but as parallel characters they used to be good friends, with Dick Grayson coming up with the name of Ollie's "Arrow-Cave". Then later they had a falling out over political differences and Green Arrow's overreliance on trick arrows. After that it was usually more a case of them disagreeing with each other as to why to do things, but still trusting each other to get things done.

The Forehead yes. In one other storyline the Joker thought he finally killed Batman and the result was Mr J going sane. He became normal as all get out. So we're suggesting that the opposite takes place here.

Well they already gave The Batman Who Laughs his backstory. Though it's basically the same as the Killing Joke... but went even further. Joker, somehow more insane than ever, kidnaps families of three. Then after trapping Batman, he blows up several hospitals then begins recreating how Batman's family died. Onpy this time, after killing the parents, he infected the child with his Joker Toxin. This went over and over until Batman broke Joker's neck. When Joker died Batman was infected by a more powerful Joker Toxin strain. He then killed the Bat Family, then went after the League and made Superman and Jon kill Lois Lane, then the world came last. Barbatos (I like to think he's the Dark Batman Version Shazam) invited the Batman Who Laughs to be the leader of the evil Batmen.
