The best thing is, it's not staged. On the bonus material of the webcast/audio drama "Death comes to Time" there is an interview with Silvester McCoy and he references this very incident. He says that he really thought it was Tom^^

Actually Tom and Sylvester have been on Dr. Who panels together. You can find them on KZbin or the BBC Dr. Who site. Knowing Tom he would of had know problem talking to Sylvester or anybody else. Looking at the way Tom interacts with the fans I think he just likes to find out about and talk to people. Hoping I meet both these gentlemen before we lose either one.
